Lunch dates are dedicated to short outings for lunch at a selection of restaurants. The transport fare is included in the cost of the outing, but the cost of lunch is the client’s responsibility.

WEEkLY DAY TRIPS 9am to 3.30pm

Our weekly bus trips include outings to a variety of destinations. These might include drives in the country, a trip to the beach for fish and chips, mystery tours or lunch and entertainment at a club. The costs are dependent on the distance travelled and cover the cost of transport and morning tea. The cost of lunch is the responsibility of the customer. While a lunch venue is always included, clients are welcome to bring their own lunch.

Other costs may be involved from time to time to cover entrance fees or shows, however prior notice will be given when this happens.

WhAT DOES ThE SOCIAL SuPPORT PROGRAm OFFER?The Social Support Program provides assistance in a range of services. These include friendly home visits, social functions, day trips, shopping assistance and helping with referrals to other services when needed.

ACTIvITY INFORmATIONMonthly calendars and itineraries are posted to all clients each month. The calendars assist clients to plan which bus trips or outings they want to attend.

GROuP SOCIAL FuNCTIONSGroup social functions are held on a regular basis and include themed events such as a carnival based around the EKKA, a Melbourne Cup luncheon and a Christmas party.

The cost for these events includes transport, meals and entertainment. Invitations to these special events will be mailed prior to the event.

vOLuNTEER vISITINGThe Social Support Program provides an in-home visiting program for clients who are house-bound or who don’t feel comfortable in a group setting.

Customers and volunteers are matched according to their interests and visits take place on a weekly needs basis. Short outings such as morning tea at a coffee shop, a visit to the library, movies or a local park can be arranged in the customer’s local area.

ShOP ASSISTShop Assist is a complete door-to-door service provided to clients who are unable to access a shopping centre without help.

Transport to and from the client’s home is included. Every individual’s needs are taken into account and the service is tailored to meet them.

This service can help you reach items on shelves, do the banking, pick up prescriptions or pay bills.

ELIGIbILITY CRITERIATransitCare is a community organisation funded by both the Commonwealth and State Governments.

To be eligible for the service, clients must meet the following criteria:

• be a frail older person with functional limitations as a result of a moderate, severe or profound disability

• be an unpaid carer of an frail older person

• live in the community

• have difficulty performing activities of daily living without help due to functional limitations

• be at risk of admission to long-term residential care.
