and Awards Celebration

OUTSTANDING CRAFTSPERSON AWARD

The Outstanding Craftsperson Award gives every CCS Association member company the opportunity to publicly honor a craftsperson for their safety efforts. Each company may determine their own criteria for the awards. Every person whose application is received will win the award; however, each company is limited to only one honoree. The awards will be announced at the 25th Annual CCS Anniversary and Awards Celebration on Wednesday, May 2, 2018 at the Indiana State Museum. Winners will receive a Columbia Watertight II jacket embroidered with the CCS logo along with “Outstanding Craftsperson 2018” and the recipient’s name. Winners and their families will also be invited to a special event at an Indianapolis Indians game in June. All nominees should be present at the Awards Celebration so that they will be able to be recognized and accept the award in person. Special Instructions

▪ Please be sure to provide the proper spelling of the nominee’s name.

▪ Please be sure to provide a color photo of the nominee. (Photo will be displayed during the award announcement.)

Payment

▪ Companies are asked to submit $100 to cover the cost of the jacket.

▪ Check can be made payable to CCS and mailed to CCS, 8909 Purdue Rd., Suite 130, Indianapolis, IN 46268.

▪ Credit card payments can be made by calling the CCS Office at (317) 686-2665.

Eligibility and Submission Requirements

▪ Nominee must be from a CCS Association member company.

▪ Each CCS Association member company may only recognize one craftsperson.

▪ Nominations can come from another individual or be a self-nomination.

▪ Submission must include: award application in PDF format and color photo of nominee in .jpg format.

▪ Companies are asked to submit $100 to cover the cost of the jacket.

▪ Entries must be emailed to awards@mattisoncorp.com.

▪ Submissions must be received no later than Friday, March 23, 2018 at 5 p.m. EDT.
