Ultimately we are interested in seeing a hard zero on chronic homelessness
However, this is how we will measure achieving functional zero chronic homelessness
<
chronic homelessness
accounts for less than 0.1% of your
actively homeless number as measured by
your quality By-Name List**
chronic homelessness
accounts for 3 or less people of your
actively homeless as measured by a quality
By-Name List**
* Whichever is greater** Must also be sustained for three months

OUTFLOW: MOVED TO INACTIVE
Start by tracking monthly housing move-ins. As Canadian communities build a real-
time, By-Name List, you’ll track and respond to a more dynamic picture of your
full systems and report By-Name List measures for Chronic.

• For information on By-Name Lists and to access scorecard
materials (and coordinated access information), go to
www.20KHomes.ca and under “Resources” you will find the By-
Name List page
• The Scorecard Scoring Guide outlines what the BNL Scorecard
is, its purpose and how to use and score it (some updates
coming)
• The on-line BNL Scorecard is where you submit your Scorecard
to the Campaign - just check the boxes and it immediately
updates our database and your Performance Management
Tracker

• We will be asking communities to submit the BNL
Scorecard quarterly until a 10/10 has been achieved
• We will then review your BNL worksheet with you and
look for three months of balanced data from your BNL
measures as part of your monthly reporting
• You will then have achieved a Quality By-Name List!
• Our goal is to have all Campaign communities at a
Quality By-Name List by 2019

• Its not to judge
• It’s a self-assessment quality improvement tool to
support you in your efforts to reach zero on chronic
homelessness
• Sharing it with the Campaign allows us to: o understand how we might best be able to support
communities moving forward
o celebrate progress of Campaign communities reaching
key milestones
o have confidence that we are all working with a similar
level of quality data to monitor progress on our shared
goal of functional zero on chronic homelessness
