To complete your 2020 Open Enrollment, access Workday using one of the following options: Access Open Enrollment with ease through the Workday Mobile App! 2020 Open Enrollment Instructions: Workday

Welcome to 2020 Open Enrollment! Elections you make during Open Enrollment will

become effective January 1, 2020.

To complete your 2020 Open Enrollment, access Workday using one of the following

options:

• Access Open Enrollment with ease through the Workday Mobile App! If you do

not have the Workday Mobile App, scan the QR code below and enter

organization id: convatec

• Log into Workday directly through https://www.myworkday.com/convatec

If signed on the VPN, select “I have a ConvaTec email address”.

If you do not have a ConvaTec email address, Select “No CVT email or New Starter”. You

will then use your credentials that have been provided to you.

If you need assistance logging in to Workday, email to [email protected].

From the home screen, click on one of the “Inbox” icons to access the Change Benefits for Open

Enrollment.

Enrollment Tips and Reminders

• 2020 Benefits Enrollment Dates: October 14th – November 1st

• Review your dependents in Workday. If adding new dependents during open

enrollment, have their names, dates of birth, and SSN handy.

• If enrolling in the CIGNA CDHP with HSA medical (Plan 1), you will receive the Accidental Injury – enrolled in CDHP and Hospital Indemnity – enrolled in CDHP Plans for employee only coverage at no cost to you. You may buy up for the additional coverages (employee + spouse, employee + children, or employee + family).

To complete and submit your enrollment changes, you will be required to select the minimum of employee only coverage for both the Accidental Injury – enrolled in CDHP and Hospital Indemnity – enrolled in CDHP Plans.

• If enrolling in the CIGNA CDHP and you do not wish to contribute to the HSA for 01/01/2020, you must elect the HSA and enter $0.00 to receive the employer contribution and complete your enrollment.

• If electing the CIGNA OAP PPO (Plan 2) or opting out of medical and would like to enroll in the Hospital in the Accidental Injury Plan and/or the Hospital Indemnity Plan, you must select the non-CDHP Plans.

• FSA and HSA contribution elections do not carry over. You must actively elect your HSA election and/or FSA election for the 2020 benefit plan year.

• To ensure that your enrollment has been processed, you must review, click ‘accept’, and submit at the end of your enrollment. You will see Success, You’re Enrolled with the option to view your 2020 Benefits Statement.

• To view or change your previously submitted benefits elections, log onto Workday, click on Change Open Enrollment.

Review your current benefits electrons. You will need to click on manage for each benefit plan to view and/or make changes to your elections.

Review and enter your medical plan selection for medical by clicking on manage.

Select medical plan. Click on Confirm and Continue.

Review dependents covered under medical. If dependents are accurate, click Save or modify as appropriate.

Review and enter your medical plan selection for dental by clicking on Manage.

Review and enter your dental coverage selection. Click on Confirm and Continue.

Review dependents covered under dental. If dependents are accurate, click Save or modify as appropriate.

Review and enter your vision plan selection for dental by clicking on Manage.

Review and enter your vision coverage selection. Click on Confirm and Continue.

Enter your selection for the new Hospital Indemnity Plan. If enrolling in the CIGNA CDHP with HSA medical (Plan 1), you will receive the Hospital Indemnity – enrolled in CDHP Plans for employee only coverage at no cost to you. Click on Enroll to make your selection.

If enrolling in the CIGNA CDHP with HSA medical (Plan 1), select Hospital Indemnity Enrolled in CDHP.

If enrolling in the CIGNA OAP PPO (Plan 2) or opting out of medical and would like to enroll in the Hospital Indemnity Plan, select the CIGNA Health Hospital Indemnity Plan.

Click on Confirm and Continue.

If enrolling in the CIGNA CDHP with HSA medical (Plan 1), select Accidental Injury Enrolled in CDHP.

If enrolling in the CIGNA OAP PPO (Plan 2) or opting out of medical and would like to enroll in the Accidental Injury Plan, select the CIGNA Health Accidental Injury Plan.

Click on Confirm and Continue.

Select your coverage level, dependents, and click on Save.

If enrolling in the Cigna CDHP w/HSA (Plan 1) and you would like to contribute to the HSA, you will

need to elect to participate AND enter the amount you would like to contribute. If you do not wish to

contribute to the HSA at this time, select “elect” and enter $0.00.

Click on Enroll to make your HSA

Please Note: You can log into Workday at any time during the year to contribute to the HSA

Enter the amount per pay and annual amount you would like to contribute and click on Save..

For employees under the age of 55, the 2020 IRS limit is $3,550 (employee only) and $7,100 (all other coverage tiers).

For employees over the age of 55, the 2020 IRS limit is $4,550 (employee only) and $8,100 (all other coverage tiers).

***Please note, the above limits include the employer contribution of $500 for employee coverage and $1,000 for all other coverage tiers.

If you would like to contribute towards any of the Flexible Spending Accounts: Health Care

FSA/Dependent Care FSA/Limited Purpose FSA, you will need to elect to participate as well as enter

your contribution elections and Save.

Please note: If you have enrolled in the CIGNA OAP CDHP with the HSA, you will NOT be permitted to

contribute to the Health Care FSA. You may, however, elect to contribute towards the Limited

Purpose FSA to be used for dental and vision expenses.

ConvaTec provides Basic Life Insurance and Accidental Death and Dismemberment Coverage at 2x’s

your base salary. You may elect additional Optional Life Insurance, Spousal Life Insurance, and Child

Life Insurance***You must be enrolled in Optional Life Insurance to elect Spousal and/or Child Life Insurance.

*** Changes from your current benefit election may require completion of Evidence of Insurability

(EOI)

To make changes to your optional life insurance or to update beneficiaries, click on Manage to access

your benefit coverage, select, and click Confirm and Continue.

Select optional life insurance amount.

Review beneficiaries. You must enter your beneficiary designation for Basic Life Insurance, Accidental

Death and Dismemberment Coverage, and Optional Life Insurance. You may designate both primary

and contingent beneficiaries. Both primary and contingent designations must equal 100%.

To Edit Beneficiaries click on beneficiary box and choose from the following options.

Click Save once election and beneficiaries are accurate.

Confirm dependents and Save.

If you wish to enroll in the LegalEase legal plan, select Manage, elect to enroll, or waive to waive coverage.

Once your election has been entered, please click on Confirm and Continue, and Save.

Step 8 of 8

Please review your enrollment. If you agree with all your selections, scroll down to the bottom of the page and

click Review and Sign to access your enrollment summary.

Review Your Enrollment Summary, Click “I accept”, Submit,

and print a Copy of your Enrollment. Click on Done to Finalize.

enrollment in the 2020 Benefit Plans!
