We are grateful to the following whose generosity has helped make a difference in the quality of life of those we serve.

Greetings:

To reflect upon the growth of WestMass ElderCare over the past 43 years brings back many wonderful memories. Most cherished among them are the personal stories of individuals whose lives became easier by realizing their dream to remain at home. Countless families struggle to help their loved one’s desires come true. We are again pleased to report that our collective efforts have made a difference in the lives of so many.

During Fiscal Year 2017, our staff were proactive in building community partnerships that assist individuals moving from one care setting to another. This past year realized a strengthened relationship with the Holyoke Medical Center that assists individuals move with greater ease from hospital to home. In many instances this new protocol assured that in home supports were received by consumers with less delay aiding in faster recuperation and independence.

Another community partnership resulted in the formation of the Care Alliance of Western Mass, of which we are a member. The eight agency CAWM was responsible for submission of a successful proposal to MassHealth for certification as a Long Term Support Services Community Partner (LTSS CP), where as a Community Partner we will align with Accountable Care Organizations to provide in home and community based supports to individuals under 65 on MassHealth. More to come on this initiative in the new fiscal year.

Our Nutrition Program grew by adding more lunch clubs, breakfast programs, home delivered consumers and an LGBTQA Supper Club in partnership with the Holyoke Council on Aging. Our Home Care Program served some of the most frail at risk individuals and those individuals residing in nursing care facilities desiring to return home were assisted with in home supports. Our Adult Family Care and Personal Care Management Programs saw new growth. Our Senior Care Options, One Care and PACE contracts saw more individuals in need of care, as did requests for assistance in our Supportive and Congregate Housing Programs.

As we move into the new year we are excited of the collaborations that we have made and will continue to make with Accountable Care Organizations, health care facilities and other community based organizations, as we endeavor to serve the diverse needs of our region. As always, we are person-centered in our approach to care. We appreciate the continued endorsement and support of our funders, elected officials and allies as we continue to assist individuals to stay at home where they most want to be.
