2017 4-H Style Revue Narration Form

Deadline: Completed narration form must be submitted to the St. Joseph County MSU Extension office at 612 E. Main St., Centreville, MI 49032 by Friday, August 25, 2017 in order to be eligible to model in the 4-H Style Revue.

Name Club Birth Date ___/___/___ Age by January 1, 2017 Phone Please check each type of 4-H Project Exhibit you will be wearing as part of your “outfit” while modeling, then list the type of Garment/Accessory to be worn (i.e. skirt, blouse, sweater, dress, “personalized” jacket, “personalized” shoes, etc.). Sewing Knitting Crocheting Wardrobe Personalization Garment Narration: Please write a “narration” (description of your garment(s)) using the enclosed “Writing Narrations” as a guideline. Narrations should be 35-50 words in length, and written in the “third person” (i.e. “Tracy is wearing a red velvet dress”, not “I am wearing a red velvet dress”). Narrations may be neatly printed with an ink pen (with typing preferred). No pencil please! It would be a good idea to write your narration on a separate sheet of paper, and then transfer the final draft to this form. Be sure to proofread your narration at least three times before transferring to this sheet!
