BeWell Health Impact Program-NEW for 2014
Program Enhancement for Plan Year 2015
• The BeWell Health Impact Program (HIP) is a wellness enhancement to the current State of Arizona Benefit Options Plan, effective October 1, 2014-September 30, 2015.
• Specifically, HIP is designed as an incentive based employee wellness program for benefits eligible State of Arizona employees. Employees who successfully complete the program by reaching 500 activity points may be eligible to receive up to $200.
• Participants will accumulate points by engaging in a variety of wellness programs and activities, then self-reporting their progress in the Mayo Clinic Healthy Living online portal.

The Mayo Clinic Healthy Living Online Portal will provide the health assessment tool and tracking system as a component of HIP:• The online portal will serve as the database for all employee
self-reported health activities• Verification of activities will also be a function of the online portal• Only active, benefits eligible employees will have access to the
