Registration form is for both district and state STAR events All competitors must be affiliated by February 1, 2015 Complete each section as directed with event information, chapter/school
information, participant(s) information, and adviser contact information. Save as PDF file or word document and print two copies Obtain adviser signature Keep one copy for adviser’s records For district competition, send to district STAR event coordinator For state competition, send to Idaho STAR events coordinator: Jeannie Coulson

Event Information ~ complete all sections as prompted using the drop down arrows when appropriate
Chapter and School Information
Participant(s) Information~ all competitor information must be completed for each event
Special Access Needs~ complete for all competitors that may need special accommodations
Adviser Information~ all registration forms must have advisers signature and contact information
