Dear Friends, Almost Home Kids has continued to grow and develop in fiscal year 2014, as a result of our affiliation with Ann & Robert H. Lurie Children’s Hospital of Chicago last Fall, as well as the loyal support of you, our longtime friends and supporters. Our legacy of compassionate care in a home-like setting is thriving at our Naperville and Chicago homes, and our vision for expansion in Illinois, and beyond is taking shape.

We have celebrated many achievements this year, from legislative victories to the development of new programs benefitting our children. As a result of Corporate and Foundation support, physical therapists are now on-site 3 days a week at both locations working with our children and improving their mobility. Sleep studies and evaluations are now being conducted at AHK, which can dramatically impact the quality of life and outcomes for children in our care. Our dedicated Board and Committee members have supported us every step of the way, and our fundraising events have continued to reach new heights.

Our success would not have been possible without the partnership from each and every donor, legislator, corporate and civic partner and volunteer whose dedication to children facing incredible challenges is unwavering. We rely upon our community to advocate for children with medical complexities and sustain the network of support they so desperately need. We are so grateful for the strong foundation of support which allows us to advance our mission in these changing times.

Thank you for recognizing the importance of Almost Home Kids as a key part of the safety net in the community for children with medical complexities so that they can grow up in their own homes. The Almost Home Kids Model Almost Home Kids envisions other facilities throughout the region and beyond adopting the AHK medical model of providing high quality care close to home and strong local community support.

All key community and policy stakeholders are fully committed to the AHK model and recognize it as the premiere community-based standard of care for children with medical complexities.

How We Do It

Transitional Care — A bridge from hospital to home

AHK provides a 120-day Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nursing support from skilled pediatric nurses. AHK trains families on how to both care for their child who are medically fragile and how to prepare the home.

Respite Care

The Respite Care Program at AHK offers children a comfortable place to stay with access to medical and nursing support while their parents/caregivers tend to personal or urgent family issues. The Respite Transportation Program provides roundtrip transportation for children from their home to an AHK facility.

Our model for providing transitional and short-term respite care can be replicated throughout the country. Where the need exists, the organization will help facilitate development of other Almost Home Kids locations.

Community Support

In addition to training and care, AHK provides community resources and activities that become a support system the family can depend on for years to come.

Our Kids and Our ImpactAlmost Home Kids cares for children with medical complexities, with a range of symptoms, abilities and treatment needs. Below are just a few statistics from our Program Committee’s Outcomes Report describing the children cared for at both AHK locations in 2013:

■ 80% of families report income as “Extremely Low” < $22,550

■ 96% of children receive Medicaid

■ 53% of children have a tracheotomy

■ 67% of children receiving transition care are prescribed 7 or more complex medications per day

Almost Home Kids train parents/foster parents on how to confidently care for their children at home. The following outcomes are reported from caregivers in 2013:

■ 0% of children in our transitional care program were readmitted to the hospital for a failed home health care plan 30 days post discharge.

■ 75% of individual parents/foster parents experience a reduction in stress as a result of receiving services at Almost Home Kids.

■ 100% of parents/foster parents taking their child home develop competency in operating medical equipment and medication administration.

New Program Highlight: Sleep MedicineAlmost Home Kids Sleep Medicine program was launched in 2014. Thanks to AHK’s affiliation with Ann & Robert H. Lurie Children’s Hospital of Chicago, and under the leadership of renowned sleep medicine specialist Dr. Stephen Sheldon, along with the Lurie Children’s sleep medicine team, AHK is now able to provide onsite evaluations and overnight sleep study tests. AHK provides a safe and cost-efficient alternative to an overnight stay in hospital for children that have been recommended to be weaned from their ventilator or decanulated from their trach as an alternative to an overnight stay in the hospital. Thanks to this program, and because of the limited availability of pediatric intensive care beds in the Chicagoland area, Almost Home Kids offers a solution to this need.

“Our Sleep Medicine Program brings Dr. Sheldon, one of the preeminent sleep medicine experts, right to our children” said Debbie Grisko, AHK President & Executive Director.

“Not only does this allow sleep studies to take place for a population that desperately needs them, but can potentially result in a significantly improved quality of life for the child, in addition to tremendous cost savings to the Medicaid system.”

Did you know?■ Almost Home Kids has more than 1600 Fans on Facebook, and 223 Twitter followers! We share pictures

and stories of our amazing children daily, and we love hearing from our friends and supporters. Keep “Liking” our posts!

■ This year our recreational volunteers spent more than 3780 hours rocking, reading and playing games with the kids at our Naperville and Chicago homes. Volunteers make our house a home and provide so much love and care to our kids.

■ Languages spoken at our homes this year include English, Spanish, American Sign Language, Urdu, Polish, Russian, Gujarati, Arabic and Mandarin!

Our CollaborationsAHK relies on the generosity of corporations and organizations to sustain our mission and make our house a home. Employee and community groups play an integral role in maintaining and beautifying our grounds and facilities. Below are just a few examples of businesses and educational institutions that have gone above and beyond in support of our children and programs this year.

■ Thanks to our friends at UPS, our children and staff now have a comfortable and safe outdoor space! A dedicated volunteer group of UPS employees donated the resources and their time several weeks in a row and transformed a rarely used shed in our back yard into a serene screened-in gazebo, complete with new screens, electricity, ceiling fans, skylights, a new roof and landscaping around the structure, creating a space to relax and enjoy the outside.

■ Data collection and tracking is a big challenge for small organizations like Almost Home Kids. Thanks to a huge in-kind donation of time and talent from our friends at PricewaterhouseCoopers (PwC), AHK now has a custom designed financial/program tracking and reporting system so that we can better quantify our model of care.

■ After just one visit to our Chicago facility, Magellan Corporation, A Sea Trade Company, committed to being the Presenting Sponsor of our 3rd Annual Share the Care Ball! This lead gift made a tremendous impact on the overall record fundraising from this event.

■ The DreamHome at the Merchandise Mart has named Almost Home Kids as their charitable beneficiary for the past three years. Through this collaboration the interior design community has embraced our mission and our children, and together they have raised awareness and over $75,000 in support.

■ DePaul University partnered with AHK this year in a very big way…we now have DePaul University Nursing School students committed to serving AHK in a volunteer capacity for a two year practicum. These students will make a significant impact on the children they volunteer with and will gain a unique educational experience learning specialized care for children with medical complexities.

■ For more than five years, AHK has been a practicum site for students of the Lewis University School of Nursing. Our clinicians are proud to share their unique experience with children with specialized healthcare needs with this new generation of nurses.

Nigel Einhaus was referred to Almost Home Kids in 2013 by OSF St. Francis Medical Center in Peoria. He stayed with us for 39 days and his parents received the necessary training and care planning assistance to successfully transition him home at 4 months old. He is now a bright and healthy toddler and doing great at home in Ladd, Illinois!
