The primary aim of radiation therapy treatments is

to deliver an optimal dose to the target volume while

minimizing dose to the surrounding normal structures.

Although increasingly complex plans are being employed

to achieve this clinical goal, there remain a number of

challenges posed by the inaccuracies of treating patients,

where tumors and critical structures are constantly moving

and adapting with normal physiological functions.

Firstly the respiratory cycle of the patient results in large

inaccuracies being introduced into both the planning

and delivery of radiation treatment, especially in the

abdominal and thoracic regions. Secondly, the anatomical

positioning of critical structures relative to the target

volume can be such that the prescribed dose cannot be

delivered without an unacceptable volume of critical

structure being irradiated, such as the heart for left-sided

breast treatments.

Various methods have been designed to gain control of

respiratory motion. However the best clinical solution will

be able to easily cope with a number of unique clinical

scenarios, such as applying simple and efficient breath

holds at an applicable threshold level.

To manage these challenges Elekta designed Active

Breathing Coordinator™, a solution enabling accurate

and reproducible breath holds to be repeatedly applied.

It allows clinicians to pause a patient’s breathing at a

precisely indicated volume and co-ordinate treatment

delivery, or imaging with this pause.

Active Breathing Coordinator adds precision and

repeatability to the well-conceived concept of deep

inspiration breath hold. The system is entirely portable,

allowing it to be used in any treatment or imaging room.

Active Breathing Coordinator with simulation and/or CT

simulation, provides enhanced data with which to plan

more precise treatments.

ABC R2.0 Full Upgrade KitThis upgrade allows customers installed with Active Breathing Coordinator R1.02 to obtain the features included in R2.0. It consists of the following components and benefits:

Achieving Accuracy

One of the major obstacles to achieving accuracy in the delivery of radiation therapy is patient positioning and organ motion. The introduction of Elekta Synergy® and Elekta Axesse™ minimizes the uncertainties arising from target motion by providing 3D volume imaging at the time of treatment, highlighting any positional discrepancies before treatment begins. This allows clinicians to reduce margins with confidence, escalate dose and move towards hypofractionated treatment regimes. HexaPOD™ evo RT System completes the image guided radiation therapy (IGRT) localization chain by enabling accurate and remote geometric correction of any discrepancies observed and then detecting any table movement before treatment.

A Unique Patient Positioning System

HexaPOD evo RT System is a unique fully robotic patient positioning system with six degrees of freedom to correct for any misalignments detected by X-ray volume imaging (XVI) or portal imaging, provided by the Elekta range of digital linear accelerators. The computer controlled robotic treatment couch allows six independent degrees of freedom: x, y, z, pitch, roll and yaw and is able to achieve submillimeter 6D conformal positioning accuracy.

HexaPOD evo RT System comprises the HexaPOD evo RT Couchtop, equipped with the new generation homogenous carbon fiber couchtop – based on the iBEAM® evo Couchtop and iGUIDE® system – including the camera tracking system and control consoles, both inside and outside the treatment room.

Fast and Flexible Cost Saving Tool

HexaPOD evo RT System technology is compatible with a range of Elekta linear accelerators and, when integrated with the iGUIDE software, enables fast, flexible and automated patient set-up. This makes it a time and cost saving tool for any modern radiation therapy department.

iGUIDE® Tracking System and iGUIDE Software

The intuitive user interface of the iGUIDE software uses clinical workflows to aid the user and supports accurate positioning of the HexaPOD evo RT Couchtop to the defined isocenter co-ordinates, relative table position or a new position based on correctional data received from XVI, using all six degrees of freedom. The iGUIDE software is accessible inside and outside the treatment room.

HexaPOD evo RT System can also be controlled manually by using the handheld control when required.

The iGUIDE Tracking System along with the iGUIDE software controls the robotic couchtop and validates the table position. The high-precision camera tracks the markers on the reference frame in real time, making it possible to calculate the position of the robotic couchtop and patient.

HexaPOD™ evo RT System can interface to XVI where image registration is carried out, which provides the table correction vectors required to compensate for any observed organ, or patient movement. All table correction movements can be remotely performed by HexaPOD evo RT Couchtop and no additional table movements are required to correct for any misalignment.

The iGUIDE Reference Frame is placed over the patient and the patient is aligned according to their tattoos.

Once aligned to tattoos, the iGUIDE software registers the patient and then the relative table movements are made.

Operator selects the Unload position at the HexaPOD evo RT Couchtop and lowers the Precise table for the patient to stand up conveniently.

Patient receives treatment.

Select patient prescription on iGUIDE® software.

The patient lies on the treatment couch in the BodyFIX® vacuum cushion.

iGUIDE software automatically moves the HexaPOD evo RT Couchtop to the corrected isocenter position as defined by the XVI system.

An Elekta VolumeView™ image is acquired.

Elekta VolumeView image registration with reference CT scan to achieve translations and rotations required to correct the patient position.

The kV panel and collimator are moved into position.

Modular Couchtop - Uncompromised Patient Setup

New from Elekta, Connexion™ Modular Couchtop is the

next generation couchtop system based on an award-

winning modular design. Individual modules can be

easily configured to meet specific patient requirements,

making an ideal table top solution for both conventional

treatments as well as radiotherapy techniques such

as Image Guided Radiation Therapy (IGRT), Intensity

Modulated Radiation Therapy (IMRT), Volume Modulated

Arc Therapy (VMAT), and Stereotactic Radiation

Therapy (SRT).

approaches without compromising the set-up time.

With its unique modular design, Connexion removes the

restrictions of a single couchtop, allowing you to adapt

offers clinics the flexibility to select the most appropriate

couchtop providing personalized efficiency, and

strengthening the ability to optimize treatment outcomes.

A Single Solution to Meet a Range of Clinical Needs

Patient throughput is an essential part of any clinic.

Connexion allows clinics to maintain this essential

requirement with easily exchangeable, lightweight

modules by providing seamless transition of the individual

modules. As a single solution, adaptable to suit the

individual patient treatment needs, Connexion supports

a number of modules designed to fulfill specific

treatment indications.

Connexion™ - Easy and Efficient Workflow

To mount the inlay of the Connexion Central Opening Module, simply place it into the aperture of the module. To remove it, press against the inlay from below and remove it.

To remove a module, open the locking mechanism by spinning the wheel and pulling the handle on one side of the base board.

All Connexion modules are mounted and removed in the same way. Set the module at a slight angle so that the small safety pins on both sides of the base board interlock. Slide the module straight in until you hear a click and the module is flush with the baseboard.

Connexion™ Basic Components

Connexion Central Opening Module with Connexion Solid InlayBeing able to reduce attenuation in this scenario results in lower skin dose to the patient. The Connexion Central Opening Module provides a central opening where attenuation and dose build-up are minimized, allowing you to reduce skin doses. The module is supplied with a Solid Inlay.

Connexion Head and Neck ModuleProviding patient-specific immobilization for head and neck treatments often require a variety of immobilization devices. The Connexion Head and Neck Module provides a dedicated solution to support head and neck immobilization, by providing a common interface to not only Elekta immobilization devices, but also other commonly used third party accessories.

Connexion Solid InlayThe Solid Inlay is required for the Connexion Central Opening Module. It provides an area with low attenuation properties.

Connexion Base BoardThe Connexion modular couchtop consists of a Base Board that attaches to the Precise Table and several modules which can be attached (and detached) to the Base Board. Each of the different modules is designed to fulfill specific treatment scenarios.

Connexion Imaging ModuleDesigned to address the needs of IGRT, IMRT and VMAT techniques, the Connexion Imaging Module accomplishes this through its low, homogenous beam attenuation properties, providing a sophisticated solution for modern treatment techniques.

Connexion Lateral Opening Module with Connexion Short Indexing BarBy reducing attenuation and build-up doses to the skin from treatment fields that would normally pass though the couchtop, the lateral opening module is designed for techniques that use oblique fields, such as SBRT. This module increases clearance around the couchtop, giving you extra flexibility in treatment set-up when you need it. The Short Indexing Bar is included for repositioning of accessories. Basic Components

Connexion Module CaseThe Module Case protects the different modules against damage during storage.

Connexion Short Indexing BarThe Short Indexing bar is required for accurate repositioning of accessories at the middle area of the Connexion Lateral Opening Module.

Connexion Indexing Bridges Set (Set of 2)The Indexing Bridges are required in order to use the BodyFIX 14 Target Positioner together with the Connexion Lateral Opening Module for SBRT.

Connexion Removable Side Rail Set (Set of 2)The removable accessory rails are positioned along the superior lateral area of the Connexion™ Imaging Module and the Connexion™ Central Opening Module. They are designed to be simply and easily removed between treatments.

Connexion Side Rail Arc ReplacementIn some cases, the side rail at the Connexion Base Board foot end has to be demounted for reasons of space. The Side Rail Arc Replacement allows closing the holes on the Base Board.

iBEAM® evo CouchtopThe clear vision upgrade

The iBEAM® evo Couchtop is in perfect synergy with

modern radiation therapy techniques, such as IMRT,

IGRT and a range of imaging modalities. IGRT requires

high-quality kV or MV imaging to enable optimized

image registration for misalignment detection and

subsequent correction.

The iBEAM evo Couchtop is the next generation couchtop

for both imaging and treatment, providing outstanding

in situ imaging quality and minimizing artifacts.

Conventional couchtops offer dedicated treatment areas

with relatively low attenuation. However, these often

contain metal and other solid support structures that

attenuate the beam creating limitations to image quality

and beam angles. The iBEAM evo Couchtop unique

homogenous carbon fiber sandwich design contains

no metal in the treatment area and offers improved

radiotranslucency with a minimized attenuation spread

across the range of beam entry angles, providing the

perfect solution for IMRT, VMAT and IGRT.

The improved homogenous construction of the iBEAM

evo Couchtop provides high rigidity and strength, thus

eliminating local patient sagging (as can occur with “tennis

racket” inserts) and permitting increased patient load of

200 kg (440 lb.) with less table flex. The integrated indexing

system optimizes patient positioning accuracy and

reproducibility, reducing patient setup time, minimizing

patient setup inaccuracies and maximizing the stability

of positioning devices.

The iBEAM evo Couchtop interchangeable extensions

are light, easy to attach and enable flexible patient

positioning, rapid patient setup and improved clinical

workflow.

The iBEAM evo Couchtop also addresses the clinical needs

of intracranial and extracranial stereotactic treatments,

providing a solid foundation for stereotactic hardware

(e.g. BodyFIX® and HeadFIX® stereotactic frames and

Fraxion™ frame).

The iBEAM® evo CT Overlay facilitates new potentials for

treatment planning and simulation.

The iBEAM evo CT Overlay is identical in design, geometry

and dosimetric properties with the iBEAM evo Couchtop

and offers the total departmental solution in combination

with an iBEAM evo Couchtop (installed on the linac table).

The iBEAM evo CT Overlay rests above the plane of the

original CT cradle. The digital removal of the cradle

provides the capability to take into account the beam

attenuation and dosimetric aspects of the iBEAM evo

Couchtop in your treatment planning data set.

By utilizing the same tabletop configuration for planning

and treatment, both patient position and beam modeling

can be accurately represented and replicated.

The integrated indexing system optimizes patient

HeadFIX Simple Steps

Carefully place the patient’s head on the vacuum cushion, insert the mouthpiece into the patient’s mouth and mold the vacuum cushion around the back of the patient’s head.

Set up the stereotactic frame and lock the quick-release clips.

Set up the HeadFIX Base Plate on the table top.

Position the HeadFIX Vacuum Cushion between the posts and index it to the base plate.

Precision treatments require accurate, reproducible patient

positioning and reliable immobilization. HeadFIX® is a

vacuum-activated head frame system, specifically designed

to overcome the drawbacks of conventional invasive

fixation and non-invasive thermoplastic masks.

To achieve high-precision positioning and reproducibility,

the HeadFIX system uses a mouthpiece with the patient’s

upper palate impression and solid vacuum bonding with

the hard palate. The patient-specific dental mold is quickly

and easily formed, using a dental impression material

and the HeadFIX Mouthpiece. The vacuum-activated

mouthpiece immobilizes the patient.

The nosepiece is an alternative to the mouthpiece for

toothless patients and patients with mycoses in the mouth.

HeadFIX is constructed entirely of radiotranslucent carbon

fiber compatible with CT scanners, X-ray volume imaging

and portal imaging. The construction ensures rigidity and

reliability while restricting patient movement during both

imaging and treatment.

Non-invasive stereotactic reference frames are available for

both intracranial and head-and-neck regions of interest,

providing precise target localization and patient setup for

imaging and treatment.

The BlueBAG HeadFIX Vacuum Cushion provides additional

patient comfort while ensuring cranial and head-and-

neck support.

Reposition accuracy of bite block based systems was never as good as expected. The problem therefore with bite block based systems was mainly the difficulty in establishing an objective, identical and reproducible connection of the impression tray to the upper palate. The key feature of the HeadFIX system is its Mouthpiece with the patient’s upper palate impression. The custom formed Mouthpiece is drawn to the patient’s maxilla using a vacuum process. It is fixed without the active cooperation of the patient, and control of the vacuum pressure ensures adequate immobilization and accurate repositioning.

The HeadSTEP iFRAME based immobilization system

provides one of the easiest to use devices available on the

market. Its excellent robustness and quality guarantees

precise and simple repositioning in routine cranial as well

as head, neck and shoulder immobilization. The HeadSTEP

system is designed to include the iCAST thermoplastic

material and the disposable iFRAME. The iFRAME allows

easy molding and symmetrical pull-down by one therapist.

The joint of the wing-shaped iFRAME is one of the great

advantages of the system, combining the total coverage

of the anterior, lateral and superior head regions with the

iFRAME locked in the shaped channels, minimizing the

rotation of the head in all three axes.

The low-temperature iCAST thermoplastic is available in

a variety of perforation patterns. Its rigidity, resistance to

shrinkage and easy molding quality is outstanding.

The HeadSTEP system uses a 23-step elevation mechanism,

which permits immobilization at variable head angles

without the use of wedges. It can be used on all

commercially available couches and is also indexable for

enhanced reproducibility.

Head and Neck immobilization

Mold around the chin andthe eyes, advising the patient to keep their eyes closed. Let the mask cool off for 5 minutes until it is really hardened.

Snap the frame ends in the baseplate and lock it at the back.

Mold the mask materialaround the patient´s facialoutline. The molding of thenose bridge is of crucialimportance, as this is thereference point of the mask.

Tilting slightly downward at the chin, gently and steadily press the thermoplastic down, over the patient’s face, until the outer frame touches the baseplate.

The patented BodyFIX dual vacuum technology maximizes

repositioning accuracy and intra-treatment patient

stability by reducing both involuntary and voluntary

Manufactured entirely from radiotranslucent materials,

BodyFIX provides artifact-free image clarity with minimal

beam attenuation. The unique cover sheet nestles around

the patient and produces a uniform pressure, securely

immobilizing the patient’s body parts.

The immobilization system requires only one radiation

therapist for first and daily patient set-up.

The BlueBAG BodyFIX Vacuum Cushions create a

position for up to six weeks. They can be used for

Switch on the vacuum pump connected to the BodyFIX BlueBAG vacuum cushion to generate a vacuum and mold the cushion around the patient‘s contours.

Position the arc of the diaphragm control and the appropriate plate over the diaphragm (above the cover sheet), below the ribs.

Position the BodyFIX BlueBAG vacuum cushion on the imaging or treatment table using the indexing bars (prerequisite is the BodyFIX 14 indexing system).

Position the patient on the BodyFIX BlueBAG vacuum cushion.

Position the manifold tube between the patient’s legs to provide optimal distribution of the air flow. Then place the manifold cushions over the patient.

Cover the patient with the cover sheet, making sure it is accurately attached with the adhesive tape and switch on the BodyFIX Vacuum Pump connected to the manifold tube.

Position the patient on the BlueBAG BodyFIX vacuum cushion according to the region of interest.

Position the manifold tube between the patient’s legs to provide optimal distribution of the airflow. Then place the manifold cushions over the patient.

Cover the patient with the cover sheet, making sure it is accurately attached to the BlueBAG BodyFIX and switch on the BodyFIX vacuum pump connected to the manifold tube.

Position the BlueBAG™

BodyFIX® vacuum cushion on the imaging or treatment table using the indexing bars.

To generate an optimal mold of the patient, firmly pull on the cover sheet.

Switch on the vacuum pump which is connected to the BlueBAG BodyFIX and generate a vacuum between 600 and 650mbar.

Set the required vacuum level for the immobilization of the patient. Patient is now ready for imaging and/or radiation therapy.

Remove the BlueBAG BodyFIX from the tabletop and store safely for subsequent fractions.

WingSTEP™ enables easy positioning, precise repositioning

and high patient comfort during breast and thorax

treatment. With its excellent robustness and quality, this

patient friendly and non-invasive positioning system is a

novel device designed to improve the accuracy of radiation

therapy for the breast, thorax and upper abdomen areas.

The WingSTEP comfortably positions and supports the arms

above the head in supine position. The new system allows

natural movement of the upper and lower arm, at the same

time guaranteeing very precise, comfortable and easily

reproducible positioning. The head cushion is indexed,

assuring a reproducible patient position throughout the total

course of treatment.

The WingSTEP, especially adapted to fit through all common

CT gantries to achieve precise planning, simplifies and

BreastSTEP™ guarantees high comfort and simple

positioning, assuring a reproducible patient position

throughout the total course of treatment. The BreastSTEP

is designed to improve the accuracy of radiation therapy

for the breast, thorax and upper abdomen areas in a

patient friendly and non-invasive manner.

BreastSTEP consists of a base plate and an upper plate

made of a low-density foam core covered with thin layers

of carbon fiber. It allows the beam to pass through the

BreastSTEP from any angle with minimal attenuation.

Further features of the BreastSTEP are the indexed

aperture for placing head supports, the bottom stop and

the adjustable height arm and wrist supports, allowing

comfortable, easy, customized patient positioning.

BreastSTEP can be positioned at four different upper

heights, 18 upper arm and eight lower arm options which

comfortably position and support the arms above the head

in supine position.

BreastSTEP can be used on all brands of couches available

BellySTEP™ foam based positioning system provides

one of the most comfortable devices available on the

market. The three different size belly inserts guarantee

customized bowel displacement. This allows high precision

repositioning in routine pelvic and belly positioning.

BellySTEP is designed to reduce the irradiated small bowel

volume of patients to facilitate a proper and comfortable

prone position during pelvic area treatment.

BellySTEP consists of a foam based baseplate covered

with artificial leather, interchangeable belly inserts and

the BellySTEP prone pillow. All BellySTEP parts are made

of polyethylene foam which offers patients high comfort

without compromising stability.

BellySTEP allows for patient friendly, non-invasive

positioning whenever accurate and reproducible patient

positioning is essential for optimized therapy schedules. It

simplifies and expedites the radiation therapy procedure

to increase time efficiency.

In addition, even MRI or PET planning is possible since

BellySTEP consists of high quality metal-free and non

conductive materials.

BellySTEP with Prone Pillow

Position the patient onto the BellySTEP, starting cranially and sliding into the caudal direction. The pubic bone must be positioned in the caudal aperture for an optimal reduction of the small bowel volume.

Position the patient‘s head on the prone position head support. Place the crossed arms of the patient over the BellySTEP prone position pillow.

Set the BellySTEP baseplate on the table top.

Select the appropriate belly insert and snap it into the baseplate.

ProSTEP™ Simple Steps

ProSTEP™ is designed to improve the accuracy of the

positioning and repositioning of the lower abdomen and

extremities. This system combines the advantages and the

functionality of the FeetSTEP and the KneeSTEP, which are

available in both ABS and carbon fiber quality.

By using both cushions together with the ProSTEP base

plate, performance is enhanced, especially when treating

in the pelvic area.

On the lower end of the base plate, the KneeSTEP can

be adjusted and eventually combined with KneeSTEP

elevation blocks to acquire the optimal height and

comfort.

The mounted sliding bed for the FeetSTEP allows 17

different longitudinal (1.5 cm steps) and 10 different

indexed angular adjustment positions (5° steps).

Because of the indexed positioning of the lower

extremities, the ProSTEP guarantees high precision

repositioning in the pelvic area, thus simplifying and

expediting radiation therapy. The ProSTEP base plate is

compatible with two pin indexing bars and can be used

on all brands of couches available on the market for

diagnostic and radiotherapy settings.
