• Home-based primary care for Atlanta’s low-mobility, elderly population with

chronic illnesses.•  Admission criteria: enrolled in Medicare, difficulty leaving the home to

access care and inside I-285.

• Patients receive an average of 16 visits per year from a nurse practitioner.

•  Allows our medical team to identify and treatmost illnesses in the home.

Big-To-Do February 10, 2013Snow Adventure Benefits Children’s Hospice

•2012 was the first year that the Big-To-Do was held at Snow Mountain at Stone

Mountain Park.•The event includes snow tubing, a toddler snow hill, snowman and igloobuilding, s’mores over a open fire, plus indoor entertainment and a hot snack.

•This past year more than 500 people came out to support our Children’sPrograms.

•Individual tickets: $60 each and promotional discounts will be available throughcorporate sponsors and WSB-TV Family 2 Family.
