Our Mission at Almost Home Kids is to provide transitional care in a home-like setting to children with complicated health needs, training for their families and respite care.

Dear Friends, Fiscal Year 2013 has been a year of growth and change for Almost Home Kids. With two homes now thriving in both Naperville and Chicago, we have been able to provide transitional and respite care in a home-like setting for twice as many children as we were able to care for in previous years. The expansion of Almost Home Kids translates to millions in cost-savings to the State and a great improvement in quality of life for each child and family we serve.

Collaboration has been a key component to the success of Almost Home Kids. Now more than ever we rely on our partnerships with health care providers and our corporate community partners that provide insight and expertise on our operational and programmatic needs. Our collaboration with leaders in the design, marketing, real estate, project management, legal and insurance industries, just to name a few, has helped ensure our success in the future. Each of our Board and Committee members brings a wealth of expertise and passion for the advancement of our mission, and our legislative partners continue to promote legislation that protect children with medical complexities and their families.

Collaboration has also lead to a very exiting new chapter for Almost Home Kids. We have signed an affiliation agreement with the Children’s Hospital of Chicago Medical Center (CHCMC), the parent corporation for Ann & Robert H. Lurie Children’s Hospital of Chicago.

The affiliation means that Almost Home Kids, under the CHCMC umbrella, will be able to improve care coordination for children who are medically fragile and/or technology dependent, increase efficiency and reduce costs, which will be critical as reforms in healthcare move forward. AHK will be able to offer a greater number of services to children with complex medical conditions and their families, while expansion into other geographically convenient areas will become a reality far sooner than it would have been possible before the affiliation. Lurie Children’s is impressed with AHK‘s community-based healthcare model and foresees many future synergies.

As we look to the future we are so grateful for the countless donations of time, talent and dollars that have made our success possible. Each of our donors, partners and volunteers have played a role in advancing the critical model of care that Almost Home Kids provides for children who are medically fragile. The Almost Home Kids Model Almost Home Kids envisions other facilities throughout the region and beyond adopting the AHK medical model of providing high quality care close to home and strong local community support.

All key community and policy stakeholders are fully committed to the AHK model and recognize it as the premiere community-based standard of care for children with medical complexities.

How We Do It

Transitional Care — A bridge from hospital to home

AHK provides a 120-day Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nursing support from skilled pediatric nurses. AHK trains families on how to both care for their children who are medically fragile and how to prepare the home.

Respite Care

The Respite Care Program at AHK offers children a comfortable place to stay with access to medical and nursing support while their parents/caregivers tend to personal or urgent family issues. The Respite Transportation Program provides roundtrip transportation for children from their home to an AHK facility.

Our model for providing transitional and short-term respite care can be replicated throughout the country. Where the need exists, the organization will help facilitate development of other Almost Home Kids locations.

Community Support

In addition to training and care, AHK provides community resources and activities that become a support system the family can depend on for years to come.

Medically Fragile DefinedChildren who are medically fragile are in a stable condition, but dependent on life-sustaining medications, treatments and equipment and have a need for assistance with daily-living activities.

Medical Home Model

The child is ready to be discharged from the hospital, but parents and other caregivers need to continue learning their child’s care and home support needs to be established.

Child is diagnosed with complicated health care needs during acute phase at hospital.

Transitional Care offers short-term care for the child, care coordination and training for caregivers.

Respite Care offers a safety net in the continuum of care.

Trained caregiver takes the child back home.

Parent needs to take a short break.

Our EfficienciesAlmost Home Kids has thrived in part because of our resilience and efficiency. While our children have complex medical needs, our model is simple. Our costs have remained low because of our dedication to providing the highest level of care at the lowest cost. With two homes in operation, we rely now more than ever on volunteer and in-kind support to keep our administrative costs at a minimum.

This year:

■ 86% of our costs go to programs and children; only 14% to administrative needs

■ Volunteers contributed more than 2700 hours of recreation, maintenance and clerical support

■ In-Kind donations of everything from beds and diapers to marketing and design consultation valued more than $240,000

■ Less than 2% of our children were re-admitted to the hospital within 30 days of being discharged from AHK

■ Almost Home Kids expansion to Chicago is projected to provide an additional 3,500 days of care per year. This translates to an estimated $22 million in cost savings to Illinois taxpayers with the two locations.

■ 100% of parents report a reduction of stress upon completion of the transitional care program at Almost Home Kids

This year we celebrated yet another milestone… the 1000th child served by AHK!

This October Kevin and his mother Mary Duenas came to AHK Chicago from Ann & Robert H. Lurie Children’s Hospital of Chicago for transition care to get ready for his next cardiac surgery.

Corporate and Civic CollaborationsCorporate and civic partners continue to make a tremendous difference at Almost Home Kids. Whether providing direct service or maintenance to one of our facilities or programs, or by supporting one of our events or needs with a monetary donation, corporate collaborations flourished in fiscal year 2013. Below are just a few examples of the impact made by our corporate and civic friends and neighbors this year:

■ Baker & McKenzie provided expert pro-bono legal advice and leadership this year, for which lead attorney Robert Deignan received our Share the Care Champion award at the 2013 Share the Care Ball

■ International Facilities Management Association (IFMA) – Chicago Chapter selected Almost Home Kids as their charity partner for the year which yielded more than $15,000 in event proceeds and introduced us to many new friends in the facilities management and hospitality industry

■ Lend Lease employees visit AHK Naperville for the seventh year in a row to provide landscaping and facility improvements.

■ Artist Jeff Zimmermann created an inspirational mural for the entrance to our 211 E. Grand Avenue home in collaboration with the design team from Gary Lee Partners.

We Need Your SupportWith two homes to support and a vision that includes expansion within and outside of Illinois, our need for philanthropic support has never been greater. The generosity of individuals, companies and organizations have enabled our progress to this point and will be critical for our continued success.

Caiden spent months in several hospitals and underwent multiple surgeries and treatments for a newly diagnosed brain tumor. When it was determined that he would need to travel to the Chicago area for proton beam therapy treatment, OSF Children’s Hospital of Illinois Medical Center in Peoria recommended Almost Home Kids. Caiden’s treatment and successful outcome has required collaboration between several hospitals in addition to OSF including The Jim and Trudy Maloof St. Jude Midwest Affiliate Clinic, Central DuPage Hospital and Ann & Robert H. Lurie Children’s Hospital of Chicago.

“Every single member of the staff at the (AHK) location in Naperville surprised me with their open arms and warm hearts because they made me feel like we had known each other for a long time and treated me like I was a part of their family. They did an amazing job taking care of Caiden even through his roughest times and never gave up on trying to make sure he had a great time. There were some amazing volunteers that also took time to play with Caiden and that made him feel special too.
