Howdy! We are so glad that you are planning to participate in the 2011 District 8 4‐H Horse Show!  Since we are venturing out on a new path of registration on 4‐H Connect, this manual has a few screen shots and some tips to hopefully make your registration process as easy as possible!     A few important items to note:  

★ You will need to complete a separate registration FOR EACH HORSE you plan to show at the District Horse Show.   

★ There is an example entry form in the packet that you may review before you begin the online process.  Just remember, the paper form is just a guide to help you.  YOU MUST COMPLETE THE ONLINE ENTRY PROCESS TO BE OFFICIALLY ENTERED.   

★ You will still need to print, read, sign, and submit the Waiver, Indemnification, and Medical Treatment Authorization Form to your local County Extension Agent by the county due date.   

★ We encourage you to submit your payment through 4‐H Connect via CREDIT CARD OR E‐CHECK.  If you prefer to pay by personal check, however, you will need to work with your County Extension Agent for a 4‐H Club or County check to be submitted.  The Texas 4‐H Foundation (who processes the payments) DOES NOT ACCEPT PERSONAL CHECKS.  If a personal check is received, it will be returned to you.   

★ Only officially validated horses will be allowed to enter the horse show. 

 If at any time you have questions, contact your local County Extension Agent.  The agents and office staff would be happy to help you!   Good luck at the District Horse Show!  We look forward to seeing you there!

CERTIFICATION: County Extension Agent will certify on 4‐H Connect that the above named exhibitor and horse(s) have fulfilled the minimum 4‐H project requirements, agree to the provisions contained in the rules and regulations of the show or event entered, have completed the Waiver, Indemnification, & Medical Treatment Authorization Form, and are eligible to compete at the District 8 4‐H Horse Show.    

STALLS:  Stalls are available on a first come/first serve basis.  Stalls will be $10 per stall/per day.  Your entry does not guarantee you a stall!  Gates will be open for District 8 Horse Show at 8am on Monday, June 20.  No early arrivals will be allowed.    

NOTICE:  Classes 18‐22 are Futurity Classes and must have been nominated prior to May 1.

WAIVER, INDEMNIFICATION, AND MEDICAL TREATMENT AUTHORIZATION FORM  

1. EXCULPATORY CLAUSE. In consideration for receiving permission for _________________________’s participation in any and all activities of District 8 4‐H Horse Show (herein referred to as “camp”), which is sponsored by District 8 Texas 4‐H & Youth Development, (herein referred to as “sponsor”), I hereby release, waive, discharge, covenant not to sue, and agree to hold harmless for any and all purposes sponsor, The Texas A&M University System, the Board of Regents for The Texas A&M University System, Texas AgriLife Extension Service, Texas 4‐H and Youth Development Program, Texas 4‐H Youth Development Foundation, Texas A&M University, and their members, officers, servants, agents, volunteers, or employees (herein referred to as RELEASEES or INDEMNITEES) from any and all liabilities, claims, demands, injuries (including death), or damages, including court costs and attorney’s fees and expenses, that may be sustained by me/my child while participating in such activity, while traveling to and from the activity, or while on the premises owned or leased by RELEASEES, including injuries sustained as a result of the sole, joint, or concurrent negligence, negligence per se, statutory fault, or strict liability of RELEASEES. I understand this waiver does not apply to injuries caused by intentional or grossly negligent conduct.  

2. INDEMNITY CLAUSE. I am fully aware that there are inherent risks to my child, myself and others involved with this activity, including but not limited to: (1) The tendency of an animal to behave in ways that may result in injury, harm or even death to persons on or around them; (2) The unpredictability of an animal’s reaction to such things as sounds, sudden movement and unfamiliar objects, persons or other animals; (3) Certain hazards such as surface and subsurface conditions; and (4) Collisions and contact with other animals or objects.  I choose to voluntarily participate/allow my child to participate in said activity with full knowledge that the activity may be hazardous to me, my child and my property, and to the person and property of others.  Releasors voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained by the participant or any loss of damaged property owned by them, as a result of being engaged in such an activity, whether caused by the negligence of releases or otherwise.  I acknowledge there may be physically strenuous activities. I know of no medical reason why I/my child should not participate. I agree to indemnify and hold harmless INDEMNITEES from any and all liabilities, claims, demands, injuries (including death), or damages, including court costs and attorney’s fees and expenses, which may occur to myself, my child, other participants, and third‐persons as a result of my/my child’s participation in said activity, including injuries sustained as a result of the sole, joint, or concurrent negligence, negligence per se, statutory fault, or strict liability of INDEMNITEES.   

3. NO INSURANCE. I understand that RELEASEES may or may not maintain any insurance policy covering any circumstance arising from my/my child’s participation in this activity or any event related to that participation. As such, I am aware that I should review my personal insurance coverage. Organization may not carry general liability insurance to cover claims arising from this activity so it seeks a waiver of claims as additional consideration for the right to participate so organization, can (a) provide the activity at the lowest possible cost to participants; and (b) provide access to a greater number of participants by expending limited resources on program materials rather than on liability insurance.   

4. BINDS HEIRS. It is my express intent that this agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representatives, if I am deceased, and shall be governed by the laws of the State of Texas.    

5. MEDICAL AUTHORIZATION, INDEMNITY FOR MEDICAL EXPENSES, and WAIVER. I understand RELEASEES cannot be expected to control all of the risks articulated in this form and RELEASEES may need to respond to accidents and potential emergency situations. Therefore, I hereby give my consent for any medical treatment that may be required, as determined by a medical professional at the medical facility, during my/my child’s participation in this activity with the understanding that the cost of any such treatment will be my responsibility. I agree to indemnify and hold harmless INDEMNITEES for any costs incurred to treat me/my child, even if an INDEMNITEE has signed hospital documentation promising to pay for the treatment due to my inability to sign the documentation. I further agree to release, waive,  discharge, covenant not to sue, and agree to hold harmless for any and all purposes, RELEASEES from any and all liabilities, claims, demands, injuries (including death), or damages, including court costs and attorney’s fees and expenses, that may be sustained by me/my child while receiving medical care or in deciding to seek medical care, including while traveling to and from a medical care facility, including injuries sustained as a result of the sole, joint, or concurrent negligence, negligence per se, statutory fault, or strict liability of RELEASEES. I understand this waiver does not apply to injuries caused by intentional or grossly negligent conduct.  

 6. VOLUNTARY SIGNATURE. In signing this agreement I acknowledge and represent that I have read it, understand it, and sign it voluntarily as my own free act and deed; sponsor has not made and I have not relied on any oral representations, statements, or inducements apart from the terms contained in this agreement. I execute this document for full, adequate and complete consideration fully intending to be bound by the same, now and in the future. I understand I can choose not to sign this document and free myself and my child from its terms and the associated risks of the activity by simply not participating in the activity and choosing some other activity available to me/my child that has a lower level of risk to myself and my child. I further understand this is a voluntary, extracurricular activity. While I understand alternative activities are available to me/my child that do not have the risks associated with this activity I still desire to voluntarily engage/permit my child to engage in this activity.

Step 1.   Log onto your 4‐H Connect account.

★ Log‐on using your family’s email address & password. 

★ If you are unsure of your registered email address, contact your County Extension Agent. 

★ If you are unsure of your password, you may use the “I forgot my password” feature and the system will email you a new password. Or you may contact your County Extension Agent for assistance.

Step 2.   

Select the 4‐H Member that will be registering for the District 4‐H Horse Show.   

★ This is your Member List page.   

★ Select the 4‐H Member from the drop‐down list at the bottom of your Member List page in the “Register A Member in An Event” section. 

★ Select the Event that you need to register for.  In this case, it will be the “2011 District 8 4‐H Horse Show.”  

★ After you select the 4‐H Member and the Event, a new section will appear at the bottom on the page.   

★ Click on the “Register” button to take you to the registration screen.

Step 3.   Register for the Event by adding in the needed information, as requested. 

★ REMEMBER:  You will complete a separate entry FOR EACH HORSE.   

★ Each horse entered must have been properly validated by May 1 with your County Extension Agent.   

★ You may only enter one horse in each Qualifying Division.   

★ You may enter more than one horse in each of the Open/Invitational Divisions, but only one horse per class.   

★ The same horse may not be entered in both Western Trail and Stock Horse Trail. 

★ You must enter and participate in at least one class within a Qualifying Division at District in order to qualify for all the classes in that division at the Texas 4‐H Horse Show.   

★ The next few pages show you what that screen will look like.

Step 4.   Review your cart for check out. 

★ The amount will be displayed on this page.   

★ If you need to make any corrections, now is the time to do that by clicking the “Edit” button next to the dollar amount.   

★ Click on the “Check Out” button to submit your payment.

Step 5.   Select your payment method.

Step 6.   Confirm your payment.   

★ Click the “Confirm Order” button.   

★ This will send your registration to your County Extension Agent to certify the entry. 

★ The next page you will see is your invoice.

Step 7.   Your online registration has now been sent to the County Extension Agent for certification.   

★ You must now print, read, sign, and return the Waiver, Indemnification, and Medical Treatment Authorization Form to your County Extension Agent.   

★ You may view an invoice in “Printer Friendly Format.”  It will open up a new window. 

 

★ You may log back at any time to see if your entry has been certified at the County and/or District levels by viewing the “status” of your event registration.   

★ If you have another horse to enter in the District Horse Show, you can click the “Return To Member List” button and begin the process again for your additional horse(s).

★ Your Member List Page now shows the events you have registered for.   

 

★ In this example, you can see a few different things: 

 

★ The first registration is “Incomplete.”  If your registration status says that it is Incomplete, click “Edit” and modify and finalize your registration.   

 

★ The second registration shows that it has been “Approved.”  This means it has been approved at the final level and your entry is complete.  Do not panic if this does not change to Approved until several days after the District deadline.  You may “View/Print” your invoice at anytime. 

 

★ The third registration shows that it is “Pending at the county level.”  Before your County Extension Agent can certify your registration, you will need to supply him/her with the signed copy of your Waiver Form.     

 

★ If you have any questions, contact your County Extension Agent.
