UC DAVIS HEALTH SYSTEMUC DAVIS HEALTH SYSTEM LEADERSHIP COUNCIL SCHOLARSHIP 2010-2011The Leadership Council of the UC Davis Health System is a volunteer group of approximately 255 community leaders and ambassadors who are dedicated to supporting the mission of the Health System. As the cost of medical education continues to increase, financial assistance for students is more important than ever. The Leadership Council has a long history of supporting men and women who aspire to become physicians through UC Davis. Due to their strong interest and commitment, the Leadership Council has established the Walter Rohrer Endowed Scholarship Fund to help UC Davis medical students with their burden of debt.Leadership Council Mission StatementTo involve a variety of leaders in the Sacramento region with the UC Davis Health System to:Enhance awareness of the UC Davis Health SystemBuild an advocacy group to promote the medical school and medical centerProvide feedback on key community issues and concerns to staffBuild a base of private and public financial support for the Health SystemUC Davis Health System Mission StatementThe four-part mission of the UC Davis Health System is to teach, discover, heal, and serve:TEACHEducation - To provide current and future health care professionals with the knowledge, skills and abilities needed to provide exemplary patient care and to foster and support continued learning throughout their careersDISCOVERResearch - To advance our understanding of health and disease through basic and applied research and the translation of these discoveries into effective approaches to disease prevention, diagnosis, and treatmentHEALPatient Care - To provide access to the latest advances in medicine and consistently deliver high-quality care with competence, respect, dignity, and compassionSERVEPublic Service - To provide leadership and promote collaborative relationships with the public and private sectors to build a healthier community and regionScholarship Selection ProcessLeadership Council scholarships are awarded to UC Davis medical students based upon the following criteria:Leadership qualitiesCurrent and past involvement in volunteer and/or community outreach projectsFuture plans involving the medical professionGood academic standing, full-time enrollment, and financial need (verified by the financial aid office)The goal of the Leadership Council scholarship program is to award annual scholarships to recognize 3rd and 4th-year medical students with demonstrated leadership qualities, volunteerism history, good academic standing, and financial need. Current 1st and 2nd-year medical students are ineligible.Previous recipients of a Leadership Council Scholarship are ineligible. A student may receive a Leadership Council Scholarship for medical students one time only. Previous applicants may reapply if they meet the eligibility requirements.The chair of the Leadership Council Scholarship Committee will administer the selection process. Applicants are not personally interviewed for the scholarships. However the Scholarship Committee may contact the Office of Medical Education for further information about the applicants.All information received and reviewed by this committee will be held in confidence.Scholarship recipients will be notified by March 1, 2011 and will be expected to attend the Leadership Council Members Only Meeting program and dinner to be held on the evening of Wednesday, May 4, 2011.The Health System financial aid office will credit scholarship monies to the recipients UC Davis student account.Submission of this application is an indication of agreement to all terms detailed above.UC DAVIS HEALTH SYSTEMLEADERSHIP COUNCIL SCHOLARSHIPAPPLICATION GUIDELINES1. Applicants will apply for the scholarship through the Leadership Council scholarship program administered by the Office of Medical Education. A complete application will include the Leadership Council scholarship application form and a personal statement addressing the applicant's: Family history Scholastic background Description of how and why medical school was chosen Description of why he/she wants to be a medical professional Future plans related to the medical profession Leadership qualities and volunteerism history2. In preparing this application, students must use 14-point Times New Roman font, one-inch margins, and no more than three pages, excluding the cover page. NOTE: Applicants who do not follow these guidelines will not be considered.3. Completed applications, including cover page, photograph, and signature, must be received by the deadline. Late or incomplete applications will not be considered.4. All application materials will become the property of UC Davis Health Sciences Advancement and will not be returned.
