DESCRIPTION

EPA issues a UIC permit to PTP for Class V Injection, Permit No. NV504000001. Dated September 7, 2004. Expires Sept. 7, 2009.Issued to two permittees: Leon Mark Kizer (Allotment CC234 Owner) c/o Bureau of Indian Affairs and PTP Incorporated (Operator) for two underground injection wells: Disposal Field A and Disposal Field B, as well as the operation of up to two new Class V injection wells once the permit conditions are met.Page 14 notes that the permit is not transferrable unless notice is provided to the EPA 30 days before the transfer, and 40 CFR 144.38 is followed.Page 6 of the permit requires at least four monitoring wells for each disposal field and monitoring devices.
