ProgramGoalsThegoaloftheBiomedicalandHealthInformaticsIPhDprogramistotrainresearcherstocontributetothetranslationofbasicsciencefindingsintopatientcareandultimatelyintocommunitystandardsandpolicy.Giventhediversityofspecialtiesthatareincludedinthefieldofbiomedicalandhealthinformatics,individualtrainingwillvarydependingonthestudent’scareergoals.StudentLearningOutcomesGraduatesofthisdisciplinewillbeabletoapplyknowledgeandskillsintheareaofbiomedicalandhealthinformaticsby:
Designing,proposing,andgettingsupportfortheirresearch;Conductingresearchthatdemonstratesthehighestlevelofmethodologystandards;Performingappropriateanalysisoftheirresearchdata;and,Communicatingscientificinformationthroughresearchreportsandpublications.
ProgramDescriptionTheBiomedicalandHealthInformaticsdisciplineispartoftheUMKCPhDPrograminInterdisciplinaryStudies(IPhD).TheIPhDprogramwasdevelopedtoreflectthecomplexityandmultidisciplinarynatureofmanyoftheglobalproblemsthatneedtobeaddressed,anditisdesignedtoenablestudentstobetrainedandworkacrossmultipledisciplines.ThemissionoftheIPhDprogramistospantraditionalboundariesamongdisciplineswhilehelpingstudentsdevelopknowledgeandskillsforindependentresearchonthefundamentalquestionsofthepresentandthefuture.TheprogramprovidesopportunitiestohaveindividualizedPhDtrainingexperiencesthatreflecttheuniqueacademic,professional,andcareergoalsofeachstudent.CurriculumDescriptionStudentswithBiomedicalandHealthInformaticsastheirprimarydisciplinemustcompleteaminimumof36credithoursofgraduatecourseworkinthecombinationoftheirprimarydiscipline,co-discipline,andelectives(exclusiveofdissertationresearchhours).Nomorethan60%(21of36credithours)ofthesecoursecreditscanbefromanyonediscipline.Aminimumof9credithoursofgraduatecourseworkintheco-disciplineisrequiredbytheInterdisciplinaryPhDProgram;specificrequirementsofthestudent’schosenco-discipline,however,mayrequireadditionalcredithoursbeyondthe9.

ElectiveCourseworkThenumberofcredithoursofelectivegraduatecourseworkthatastudentcompleteswillvarydependingonthenumberofcredithoursrequiredbytheirco-discipline.Studentsmayworkwiththeiradvisorstotailorelectivecourseworkbasedontheirspecificareasofinterestandtheirchosenco-discipline.Atleast75%ofthetotalcourseworkmustbefromdisciplinesthatarecertifiedaseligibletoparticipateintheIPhDprogram.DisciplineSpecificRequirements
• QualifyingExamination:Thequalifyingexaminationmustbesuccessfullycompletedbeforethestudentcanenrollindissertationandresearchhoursandfocusonhis/herdissertationresearch.
• ComprehensiveExamination:Thecomprehensiveexaminationwillconsistofbothawrittencomponentandanoralcomponentthatreflectstheplanneddissertationresearch.
• DissertationResearch:DissertationresearchwillbeconductedwhilethestudentisenrolledinDissertationandResearch(D&R)credithours(MEDB5699).Atotalof12credithoursinD&Rmustbecompletedbeforethedissertationwillbeapproved.Dissertationresearchwillbereportedinbothawrittenformat(threepublication-readymanuscripts)andanoralformat(oraldefensewhichisaformalpresentationofdissertationresearch).
• WrittenDissertation:Thefinalformatofthedissertationworkthatwillbesubmittedtotheuniversitywillfollowgeneraldissertationformattingguidelinesandwillincludethethreepublication-readymanuscriptsinadditiontoanoverviewanddiscussionsections.
• GraduateSeminar:Studentsarerequiredtoactivelyparticipate(eitherenrolledornon-enrolled)intheMultidisciplinarySeminar(MEDB5540)untiltheybecomeaPhDcandidate(successfullycompletethecomprehensiveexamination).Theymustattend(eitherinpersonorviaBlackboardparticipation)atleast80%ofsessionseachsemester.Studentswillalsoberequiredtogiveatleastthreeresearchpresentationsfortheseminarcourse,includingatleastoneliteraturereview,oneresearchoverviewrelatedtotheirdissertationresearchtopic,andonepresentationoffindingsfromtheirdissertationresearch.

PREFACEThismanualdescribespoliciesandproceduresfordeterminingagraduatestudent'sstatusintheUniversityofMissouri-KansasCity(UMKC)SchoolofMedicine.TheseapplytostudentsinschoolofmedicinegraduateprogramsexcludingthoseenrolledintheMasterofScienceinAnesthesia&MasterofMedicalSciencePhysicianAssistantprograms.Studentsshouldbecomethoroughlyfamiliarwiththisinformationandassumeresponsibilityforknowingandcomplyingwiththeseguidelines.Allstatementsintheseguidelinesconcerningpolicies,procedures,andregulationsbytheUniversityofMissouri-KansasCitySchoolofMedicinearesubjecttochange.Theyarenotofferstocontract.TheUMKCCatalogisanadditionalsourceofinformationregardingoverallUniversitypolicies.Inseveralinstances,notedsubsequentlyinthismanual,SchoolofMedicinepoliciesdifferfrom,andsupersedeuniversitypolicies.StudentsareresponsibleforknowingtheiracademicstatusbyreferringtotheirofficialpermanentacademicrecordonfileattheSchoolofMedicineandwiththeUMKCRegistrationandRecordsOffice,andthroughofficialtranscripts.Additionalunofficialinformation,suchasschedulingandevaluationsarealsoaccessiblethroughPathwayandprogramtrackingsystems.TheresponsibilitiesoftheGraduateProgramsCommitteeonProgressionarethedevelopmentandapplicationofpoliciesandproceduresregardingstudentprogressiontowardsgraduation.ThiscommitteemakesrecommendationstotheSchoolofMedicine(SOM)GraduateCouncil.RecommendationsmadebytheGraduateProgramsCommitteeonProgressionaredesignedtoensurethepositiveprogressionofstudentswhoclearlydemonstrategrowthinknowledge,skills,andabilitiesresultinginevidence-basedcompetenciesconsistentwiththestudent’sleveloftraining.Thecommitteeisscheduledtomeetonasemesterbasistoevaluatestudentprogressionandtodetermineeligibilityforgraduationandpromotion.Thecommitteemaymeetbyemailtoconductbusinesswhendeemedappropriatebythecommitteechair.Themeetingmaybecanceledwhenthereisnobusinesstoconduct.STATEMENTOFHUMANRIGHTSTheBoardofCuratorsandUMKCarecommittedtothepolicyofequalopportunity,regardlessofrace,color,Religion,sex,sexualorientation,nationalorigin,age,disabilityandstatusasaVietnameraveteran.TheEqualOpportunityandAffirmativeActionOffice,223AdministrativeCenter,5115OakSt.,isresponsibleforallrelevantprograms.STATEMENTONDISCRIMINATION,INTIMIDATIONANDSEXUALHARRASSMENTThefaculty,administration,staff,andstudentsoftheUniversityofMissouri-KansasCityarededicatedtothepursuitofknowledgeandtheacquisitionofskillsthatwillenableustoleadrichandfulllives.Wecanpursuetheseendsonlyinacultureofmutualrespectandcivility.Itisthusincumbentuponallofus

POLICYandSTUDENTRESPONSIBILITY1. Policiesandproceduresareadoptedbythecommitteethatexplicitlydefinecriteriaformaintaining
astudentingoodacademicstandingwhileenrolledinaSchoolofMedicinegraduateprogram,including,butnotlimitedtorequiredcompetenciesandassociatedtimelinesnecessarytojustifyprogression.ThesepoliciesareminimallyconsistentwiththeUMKCGraduateAcademicRegulationsasoutlinedinthecoursecatalog.StudentsareresponsibleforkeepingapprisedoftheiracademicstatusbyreferringtotermgradesandtheirpermanentacademicrecordonfileintheUMKCRegistrationandRecordsOffice,thesecanusuallybeaccessedviaPathway.
CRITERIAFORRETENTION,PROBATION,SEPARATION,andGRADUATION:Thecommittee’srecommendationforretention,probation,separation,andgraduationfromSOMgraduateprogramsaregovernedbythefollowingcriteria:2. Retention–studentsmustmaintaina3.0(B)grade-pointaverageinallcourseworktakenfor
graduatecreditatUMKCandmustmeetanyadditionalacademicrequirementsimposedbythedepartmentorInterdisciplinaryPh.D.discipline.
3. Probation–studentswhomeetcriteriaforprobationareimmediatelysubjecttothecustomaryprobationstipulationslistedbelow,regardlessof“official”notificationfromanEducationorProgramCoordinator,orotherSchoolofMedicineprogramofficial(s).
a. WheneverthecumulativeGPAforUMKCcoursestakenforgraduatecreditbyagraduatestudentofanyclassificationfallsbelow3.0,thestudent'sstatusforthenexttermbecomes:"OnProbation-Seeprincipalgraduateadvisor."(InterdisciplinaryPh.D.studentswillbedirectedtoconsulttheirinterimadvisororresearchadvisor.)Theprincipalgraduateadvisor,interimadvisororresearchadvisorwillreviewthestudent'sprogressandprovidecounsel,andthefollowingconditionsapply:
i. Whileonprobation,agraduatestudentmustachievea3.0termGPAinordertoenrollforthefollowingterm.
ii. AgraduatestudentonprobationwhoisnotrestoredtogoodacademicstandingbytheendoftwosuccessiveregularsemestersfollowingtheterminwhichthecumulativeGPAfellbelow3.0willbedeclaredineligibletore-enroll(eligibleforseparation).
iii. Agraduatestudentonprobationwillnotberestoredtogoodstandinguntilacumulativegraduate-creditGPAofatleast3.0isachieved.
b. Anystudentretained(avoidingseparation)intheprogramregardlessofcurrentGPAisonprobation.
i. Whileonprobationthestudentmustachievea3.0termGPAii. DurationofprobationandreturntogoodstandingwillbeoutlinedbyCommittee
onProgression.
4. Separation-dismissalfromprogram.AstudentwhomeetscriteriaforseparationisinvitedtoappearbeforetheCommitteeonProgressiontopresentreasonswhys/heshouldberetainedwithintheprogram.Studentsmeetcriteriaforseparationifanyofthefollowingoccur:
a. Failuretomeetrequirementsforretentionb. Failuretomeetprobationarystipulations

c. WhenthedepartmentorschoolorInterdisciplinaryPh.D.supervisorycommittee,irrespectiveofastudent'sgrade-pointaverage,considersagraduatestudent'sperformancetobeunsatisfactory
d. Non-adherencetoprogram,school,and/oruniversitypoliciese. Failuretoreturntotheprogramfromanapprovedleaveofabsenceorfailuretoenrollin
courseworkfor2consecutivesemesters(fall&spring)withoutanapprovedleaveofabsence.
5. Graduation-Astudentwhohascompletedtherecommendedprogramofstudyandiscompliantwiththeprogram,school,anduniversitypolicies.Studentsmayelecttofulfilleitherthedegreerequirementsineffectatthetimeoftheiroriginaladmission(providedtherehasnotbeenalapseinattendanceatUMKCofmorethantwoconsecutiveterms)orthedegreerequirementsineffectatthetimeofadvisementintoaplannedgraduateprogramofstudy.a. Studentsmeettherecommendationsforgraduationifthefollowingareachieved:
i. Completionoftherequiredprogramofstudy,andii. FinalGPAatthetimethedegreeisconferred(graduatecertificates,mastersandPhD
degrees)mustbe3.0orhigherforcoursescompletedontheplanofstudy,andiii. 80%ofprogramofstudyhourscompletedwitha3.0(B)orhigher,andiv. No5000levelorhighergraduatecoursegradeofC-orlowermaycounttowardthe
degree,andv. Nocourseatthe300-or400-leveltakenonacredit/nocreditbasisandno300-or400-
levelcoursewithagradebelowB-(2.7)maycounttowardthedegreevi. Exceptionstotheabovepoliciesrequireapetitionsignedbythestudent'sadvisorand
theDeanoftheSchoolofGraduateStudies.b. Thesisanddissertationstudentsmustalsomeetthefollowingrequirements:
i. Mustfollowtheestablishedformatregulationsforthesesanddissertations(T/Ds)atUMKC,setbytheSchoolofGraduateStudies,guidedbytheGraduateOfficersCommittee.http://sgs.umkc.edu/current-students/thesis-dissertation-guidelines
ii. ApprovaloftheT/Dbythedegreecandidate'ssupervisorycommittee,whichmayincludecompletingaMaster’sPre-OralDefenseorDoctoralDissertationPre-OralDefenseandafinalT/Ddefenseofthework.ThesestepsrequirespecificformstobesubmittedtoSchoolofGraduateStudies.
iii. SubmissionoffinalappropriatelyformattedT/DtoSchoolofGraduateStudiesandnoticeofSGSReviewandCertificationofAcceptanceofT/Dprovidedtodepartment.
c. Completingthedegreerecommendationsdoesnotautomaticallyresultinconferralofthedegree.PleaserefertoUMKCCatalogforfurthergraduationrequirements.
i. Studentsmustbeenrolledinatleast1credithourduringthesemesterthedegreeisawarded.
ii. StudentsmustrequestthattheUniversityreviewtheiracademicrecordandcertifythatdegreerequirementshavebeenmet.Tomakethisrequest,studentsmustfileapplicationsforgraduationwiththeregistrarnolaterthantheendofthefourthweekofthesemesterinwhichthedegreeswillbecompleted.Studentsmustcompletegraduationapplicationduringsemesterdegreeisawarded

PROCESSANDPROCEDUREStudentsareresponsibleforknowingtheprocessandprocedureusedtoenforcethecommitteepolicies.AcademicSeparation(Dismissal)ProcessWhenagraduatestudentmeetscriteriaforseparation(dismissal),includingstudentswhohavenotbeeninpreviousacademicdifficulty,s/he:1. Shallbenotifiedthats/hemetcriteriaforseparationfromtheprogram.Thisnotificationwillcome
fromtheCommitteebyemailorinwrittenform(letter)asnecessary.NotificationwillbesenttotheEducationCoordinator,ProgramDirectororDepartmentChair,FacultyAdvisor,andGraduateCouncilChair.
2. WillbeinvitedtoattendtheCommitteemeetingtoshowcauseforretentionintheprogram.3. MayhaveadditionalinputintotheCommitteediscussionby:
a. SubmittingapersonalstatementtotheCommitteeexpressinghis/herviews.Acceptablepersonalstatementsarenomorethantwosingle-spacedtypedpageswithstandard1-inchmargins,12-pointfontandoriginalsignature.
b. Submittingsupportingdocumentsfromothers,e.g.,thestudent’sEducationCoordinator,FacultyAdvisorand/orpeers.Originalsupportingdocumentationonofficialletterheadfromoutsidesources(i.e.treatingphysician)shouldbemailedorfaxeddirectlytotheCommitteeviatheOfficeoftheCouncilonEvaluation.
TheCommitteereviewsthestudent'sperformanceandtheadditionalinputwhenapplicable,andafterthoroughdiscussionandcarefulconsideration,makesitsrecommendationforeitherretentionordismissal.IftherecommendationoftheCommitteeisforretention,therecommendationisforwardedtotheGraduateCouncilforapproval.IftherecommendationoftheCommitteeisfordismissal,thestudentisnotifiedthattherecommendationwillbereviewedatthenextscheduledGraduateCouncilmeetingandshouldtherecommendationfordismissalbeupheld,thestudent’sopportunitytoappealthedecisionwouldoccuratthatsamemeeting.ThestudentwillneedtoinformtheGraduateCouncilwhetherornots/hewishestoappealtherecommendation.ThestudentisinformedthatanappealshouldincludenewinformationormitigatingcircumstancestocausetheGraduateCounciltooverturntheCommitteerecommendation.AtthefirstscheduledGraduateCouncilmeetingfollowingtheCommitteemeetingtheGraduateCouncilreviewstherecommendationfromtheCommitteeanddetermineswhethertooverturnorsustaintherecommendationfromtheCommittee.IftheGraduateCouncilmakesadeterminationtooverturntherecommendationfordismissalfromtheCommittee,thestudentisretainedonprobationwithstipulationsspecifiedbytheGraduateCouncilorremandedbacktotheCommittee.Nopersonalappearancebyeitherthestudentoranyofthewitnessesisnecessary.IftheGraduateCouncilmakesadeterminationtosustaintherecommendationfordismissalfromtheCommittee,thestudentmay
1. choosetoforfeithis/herrighttoappeal,or2. immediately(atthesamemeeting)exercisetheirrighttoappealtothedeterminationtothe
GraduateCouncil
Ifthestudentchoosestoforfeittheirrighttoappeal,theCommitteerecommendationfordismissalremainsupheldbytheGraduateCouncilandisfinal.Ifthestudentchoosestoappeal,thens/hehastherightto:1. SubmitawrittenstatementofappealtotheGraduateCouncilinlieuofappearingpersonally.2. AppearpersonallybeforetheGraduateCouncilatthesamemeeting.

3. Requestmoretimeforappealspreparationandapostponementofpersonalappearance.a. Suchrequestmustbemadeatthesamemeetingandthestudentmustsupplyjustification
thatwillassisttheGraduateCounciltodeterminethedateandtimeofthemeetingfortheappeal..
4. Bringwitnesseswithhim/hertotestifyonhis/herbehalf.Suchwitnessesmightincludeotherstudents,facultymembers,familymembers,friendsorlegalcounsel.Atsuchahearing,thestudentispermittedtopresenthis/herstatementandsupportingwitnessesarealsopermittedtopresentstatementsinsupportofwhythestudentshouldberetainedintheprogram.Acceptablepersonalstatementsarenomorethantwosingle-spacedtypedpageswithstandard1-inchmargins,12pointfontandoriginalsignature.Proceedingsoftheseappealshearingsaredigitallyrecorded,andacopyoftherecordingisprovidedtothestudentuponrequest.
5. Neitherthestudentnoranywitnesseswhoaccompanythestudent,includinganylegalcounsel,arepermittedtocrossexaminemembersoftheGraduateCouncilorotherwitnesseswhotestify.Theadvisor/counselormayonlybepresenttoadvisethestudentduringtheproceedings.
StudentsappealingaCommitteeonProgressionrecommendationwillberequiredtosubmittotheGraduateCouncilCoordinator,1daypriortothehearingdate,acompleteandfinallistofallpersonswhohaveagreedtobepresentatthehearingontheirbehalf.TheGraduateCouncilmayacceptlatestudentsubmissionswhenlatenessisnecessitatedbythecloseproximityofthescheduledmeetings.Thislistmustprovidethenames,professions(e.g.,physician,attorney,etc.)andthespecificpurposefortheirattendance(e.g.,aswitness,advisor,etc.)atthehearing.AsignedFERPAreleasebythestudentswillberequiredatthetimeofthehearing.Afterhearingthestudentandaccompanyingwitnesses,theGraduateCouncilexcusestheseindividualsfromitsmeeting,holdsdeliberationsontheappeal,andsubsequentlydeterminesanewwhethertooverturnorsustaintherecommendationfromtheCommitteeonProgression.IftheGraduateCouncilmakesadeterminationtooverturntherecommendationfordismissalfromtheCommitteeonProgressionanditsownearlierdeterminationtosustainsaidrecommendation,thestudentisretainedonprobationwithstipulationsspecifiedbytheGraduateCouncilorremandedbacktotheCommitteeonProgression.IftheGraduateCouncilmakesadeterminationtosustaintherecommendationfordismissalfromtheCommitteeonProgressionanditsownearlierdeterminationtosustainsaidrecommendation,thentheGraduateCouncildecisionisfinal.Nofurtherappealswillbeaccepted.ThestudentwillbereferredtotheUniversityCounselingandTestingCenterforsupportandcareerguidanceTheGraduateCouncilwillnotifythestudentofthefinaldecisionbyemailorinwrittenform(letter)asnecessary.NotificationwillbesenttotheEducationCoordinator,ProgramDirector,FacultyAdvisor,andUMKCRegistrar.NON-ACADEMICSEPARATION(DISMISSAL)PROCESSTheprocedurestobefollowedwithregardtoactionstakenagainststudentsbaseduponnon-academicgroundsaresetforthintheStandardsofProfessionalConduct.APPEALPROCESSFORNon-DISMISSAL/Retained(NON-STATUSREVIEW)CASESANDPETITIONSStudentsarepermittedtoappealnon-statusreviewcasesandpetitionsthathavebeendeniedbytheCommitteeonProgressiontotheGraduateCouncilinwriting.ThiswillbethefinalappealandtheappealprocessendswiththefinalactionbytheGraduateCouncil.

PROGRAMABSENCESLeavesofabsenceshouldnotbeundertakenwithoutconsiderablethought,planningandcommunicationwithschoolanduniversitystaffandadvisors,astheymayaffectfinancialaidandmayextendastudent’sprogramofstudy.LEAVEOFABSENCEAleaveofabsenceisaperiodoftimethatastudentisnotenrolledinthecourseworkthatmakesuptheirprogramofstudy.Duringaleaveofabsence,astudentisnotparticipatingintherequirementsofhis/herprogram.Astudentmightrequestaleaveduetoanemergency,becauseofmedicalorpsychiatricillnessorduetopersonalreasons.Theschoolmightalsoplaceastudentonleaveinaccordancewithspecificacademicpolicies.Anyleavewillhaveacademicand/orfinancialrepercussions,andshouldthereforenotbeundertakenwithoutconsiderablethought,planningandcommunicationwithschoolanduniversitystaffandadvisors.Itisinappropriatetorequestaleaveofabsenceforanyofthefollowingreasons:
• Therequestismadeinanattempttoavoidreceivingundesirablegradesinoneormorecourses• Therequestismadeinanattempttogainadditionaltimetoprepareforcourseworkor
examinations• Therequestismadeinanattempttoavoidmeetingcriteriaforseparation
Retroactiveleavesofabsencearegrantedonlyinemergencysituationsorduetoadministrativeerrors.Anystudentwhodoesnotreturntothegraduateprogramwhenaleaveofabsencehasexpiredwillmeetcriteriaforseparationfromtheprogram.Astudentwholeavesagraduateprogramwithoutfollowingtheproceduresoutlinedinthispolicywillmeetcriteriaforseparationfromtheprogram.Anystudentfailingtofullyparticipateintheprogramwithoutadocumentedleaverisksmeetingcriteriaforseparation.Astudentrequestingleaveispersonallyresponsibleforensuringthetimelycompletionandsubmissionofallrequiredformsandsupportingdocumentation,andformaintainingaccuratecourseenrollmentinthePathwaysystem.Studentsrequestpermissiontoreturnfromleavebysubmittingrequiredformsandsupportingdocumentationidentifiedinthispolicy.Inordertosatisfyenrollmentrequirements,studentsmustreturntotheprogramandregisterforcourseworkfollowinganexpiredleave.ProcedureforRequestingPersonalLeave1. ThestudentcompletestheRequestforLeaveofAbsenceformandobtainssignaturesfromhis/her
StudentAffairsEducationCoordinatorandProgramDirector.2. ThestudentcompletesthecampusUMKCRequestforReturnfromLeaveformandobtainsall
necessarysignaturesfromStep2ofthatform.3. ThestudentsubmitsthecompletedRequestforLeaveofAbsencenolaterthan30dayspriortothe
anticipatedleavestartdate.4. TheCommitteeonProgressionChairwillreviewthepaperworkandrenderadecisionuponthe
request.5. IftherequestforpersonalleaveisapprovedbytheChair,OfficeofCouncilonEvaluationstaffwill
forwardtheinformationtotheRegistrar’sOfficeforfinalprocessingandsendanemailnotificationtothestudent’suniversityemailaccount.ThestudentmustthenfacilitateanynecessarychangeinenrollmentinthePathwaysystemasaresultoftheleave.
6. IftherequestforpersonalleaveisdeniedbytheChair,CouncilonEvaluationstaffwillnotifythestudentand/orrequestanyadditionalinformationnecessary.

7. Duringanyapprovedleave,astudentisrequiredtomonitorhis/herUMKCemailaddressandkeepincontactwithhis/herEducationCoordinatorandProgramDirector.
ProcedureforReturningfromPersonalLeave1. ThestudentcompletestheRequestforReturnfromLeaveformandobtainstheStudentAffairs
EducationCoordinatorandProgramDirectorsignatures.2. ThestudentcompletesthecampusUMKCRequestforReturnfromLeaveformandobtainsall
necessarysignaturesfromStep2ofthatform.3. ThestudentsubmitsthecompletedRequestforReturnfromLeavenolaterthan30daysprior,oras
soonaspossibletotheanticipatedleavereturndate.4. TheCommitteeonProgressionChairwillreviewthepaperworkandrenderadecisionuponthe
request.5. IftherequestforreturnfrompersonalleaveisapprovedbytheChair,TheOfficeoftheCouncilon
EvaluationstaffwillforwardtheinformationtotheRegistrar’sOfficeforfinalprocessingandsendanemailnotificationtothestudent’suniversityaccount.ThestudentmustthenfacilitateanynecessarychangeinenrollmentinthePathwaysystemasaresultofthereturn.
6. IftherequestforreturnfrompersonalleaveisdeniedbytheChair,CouncilonEvaluationstaffwillnotifythestudentand/orrequestanyadditionalinformationnecessary.Nostudentispermittedtoreturntotheprogramorcourseworkwithoutpermissionfromtheschool.
WITHDRAWALFROMGRADUATEPROGRAMS/SCHOOLOFMEDICINEAstudentwithdrawingfromagraduateprogramwithintheSchoolofMedicinemust:1. Submitaletter(email)ofwithdrawal(withrationaleinfluencingthedecision)totheCommitteeon
Progression,meetwiththerespectiveProgramDirectororDepartmentChair,andwillhaveanopportunitytomeetwiththeAssistantDeanofAlliedHealthorhis/herdesignee.
2. CompleteandsubmittheSchoolofMedicineProgramWithdrawalForm.Studentswhofailtosubmittheformwithin30daysafterreceiptofaletterofrequestfromtheSchoolofMedicinewillbeconsideredseparatedfromtheprogram.StudentswhonotifytheSchoolofMedicineoftheirintenttowithdrawinanotherformat(e.g.,verbal,email,note)fromtheprogramandfailtosubmittheWithdrawalFromwithin30daysafterreceiptofaletterofrequestfromtheSchoolofMedicinewillbeseparatedfromtheprogram
3. MeetwiththeeducationcoordinatortoinitiateofficialstepstowithdrawfromallcurrentcourseworkandfromtheUniversity.AstudentisnotconsideredwithdrawnfromaprogramuntilallthewithdrawalproceduresfortheUniversityhavebeencompleted.ThedatetheUniversityfinalizesastudent’swithdrawalisthedatethattheCashier’sofficewillusefortuitionreimbursement.
4. OncetheUniversityhasfinalizedastudent’swithdrawalitmaynotberescinded.
GRADUATIONGraduationProcess1. Theprocessforgraduationbeginswiththestudent,whoisresponsibleforcompletingan
applicationforgraduation.2. Eachsemesterthestudent’academicprogressismonitored,programsofstudyareupdated,and
eligibilityforgraduationischecked.3. Forstudentsmeetinggraduationcriteria,theCommitteeonProgressionwillmakea
recommendationforgraduationtotheGraduateCouncil.4. Toparticipateinthegraduationceremonyastudentmustmeetthegraduationcriteria.

GraduationProcedure1. TheProgramDirector,AcademicAdvisor,FacultyAdvisor,andCoordinatorintheOfficeof
Evaluationwillmonitorthestudents’programsofstudyandeligibilityforgraduation.2. Atthebeginningofthestudent’sfinalsemester,theprogramcoordinatorandeducation
coordinatorwillprompteligiblestudentstocompletethegraduationapplication.itisthestudent’sresponsibilitytocompleteallgraduationproceduresaccordingtotheRegistrar’spublishedschedule.
3. ThecompletedapplicationandupdatedplanofstudywillbeforwardedtotheRegistrarbytheprogramcoordinator.
4. TheUMKCOfficeofRegistrationandRecordswillauditthestudents’programofstudyformsforthedegreerequirementsandgraduateacademicregulations.
5. Studentsmustresolveanyadministrativeholds,whichmaypreventreleaseoftheirdiploma.6. Uponcompletionofthefinalsemester,therewillbeafinalconfirmationthatthestudenthasmet
allgraduationcriteriaandtheChairofGraduateCouncilwillprovidealistofstudentnameswhomeetgraduationrequirementstotheUMKCOfficeofRegistrationandRecords.
7. TheOfficeofRegistrationandRecordswillawarddegreesandreleasediplomasaccordingly.8. Studentswhodonotmeetallofthegraduationcriteria,butwanttoparticipateinthegraduation
ceremony,maysubmitarequestforexception.Onlystudentswhoarenearcompletionoftheprogram(e.g.,willachieverequirementswithinthefollowingmonth)makesucharequest.TherequestforpermissiontoparticipateinthegraduationceremonymustbesubmittedtotheChairofGraduateCouncilatthesametimegraduationapplicationsaredue.Therequestshouldbewritten(emailorletter)andincludethefollowingdetails:
