Monopolar Surgery Setup

Note: Please read the ForceTriad Energy Platform User’s Guide and all appropriate instructions for use before using the ForceTriad Energy Platform or surgical accessories.

1. Plug the system power cord into the rear panel receptacle of the ForceTriad Energy Platform and into a grounded power receptacle. (Do not use a power strip or extension cord.)

2. Turn on the ForceTriad Energy Platform and verify that the self-test is successfully completed. When the system is operational, the touchscreen appears.

Note: Do not touch the screens during power up.

3. If using a footswitch, connect it to the monopolar footswitch receptacle on the rear panel. This may require an adapter to connect a standard four-pin monopolar footswitch.

4. Apply the REM Polyhesive™ Patient Return Electrode to the patient and connect it to the patient return electrode receptacle on the front panel.

Confirm that the red REM indicator turns from red to green.

5. Connect a monopolar instrument as directed on the following pages.

Monopolar Setup InstructionsStandard Two-Button Electrosurgical Instruments

1. Connect the two-button instrument into either the Monopolar 1 or Monopolar 2 instrument receptacle on the front panel of the energy platform.

Note: When attaching a monopolar instrument, align the instrument’s connector pins with the three dots printed on the ForceTriad face plate below the monopolar receptacles. Reversing the connector will limit the instrument’s functionality and damage the connector.

Note: Each touchscreen controls the receptacle directly below the touchscreen and can only control one instrument at a time. Do not plug in more than one instrument under one touchscreen.

2. Confirm that the Standard Monopolar (Std Mono) tab appears in the appropriate touchscreen.

3. Select the power output level by touching the green up and down arrows for either the Cut or Coag mode.

4. Select the surgical mode:

a. For the Cut mode, touch either the yellow Pure or yellow Blend button.

b. For the Coag mode, touch either the blue Fulgurate or blue Spray button.

Note: If no surgical mode is selected, the Cut mode defaults to Pure and the Coag mode defaults to Fulgurate.

Valleylab Mode-Enabled Electrosurgical Devices

1. Connect the Valleylab™ mode-enabled device into either the Monopolar 1 or Monopolar 2 instrument receptacle on the front panel of the energy platform.

Note: Each touchscreen controls the receptacle directly below the touchscreen and can only control one instrument at a time. Do not plug in more than one instrument under one touchscreen.

2. Confirm that the Valleylab tab appears in the appropriate touchscreen.

Note: When a Valleylab mode-enabled device is connected, the system selects the default power zone setting for the recognized instrument.

3. Adjust the power zone setting by touching the desired gold bar.

4. While in the sterile field, move the slider control on the device to adjust power output within the selected power zone.

Note: The Cut function can be disabled by pressing the Cut power display.

Touch the green Manual button to select specific power settings not available in the sterile field control setting.

Note: The slider cannot control power when the energy platform is in the Manual Power Control mode.

Monopolar Footswitching

The ForceTriad Energy Platform provides hardware and software options for monopolar footswitching. Standard monopolar footswitching is a simple hardware configuration. Software-enabled footswitching provides footswitching capabilities for both footswitching and handswitching accessories from the Monopolar 1 receptacle.

Standard Monopolar Footswitching

1. Connect the instrument to the universal footswitch accessory port (UFP) on the front panel of the ForceTriad Energy Platform.

Note: An E0502-1 or E0017 adapter is required when using the universal footswitching accessory port with accessories that have pin diameters less than 8 mm.

2. Confirm that the Accessory (Acc.) Port tab appears in the first touchscreen.

Note: Each touchscreen controls the receptacle directly below the touchscreen and can only control one instrument at a time. Do not plug in more than one instrument under one touchscreen.

3. Select the power output level by touching the green up and down arrows for either the Cut or Coag mode.

4. Select the surgical mode:

a. For the Cut mode, touch either the yellow Pure or yellow Blend button.

b. For the Coag mode, touch either the blue Fulgurate or blue Spray button.

Monopolar Setup InstructionsSoftware Enabled Monopolar 1 Footswitching

1. Enable Monopolar 1 footswitching at the system level: Touch the wrench icon—Setup—Features—

Mono 1. The mark in the check box indicates it is enabled.

2. Exit the menus by touching the green arrows at the bottom of each screen.

3. A footswitching button with a red X is added the Standard Monopolar and Valleylab tabs in the left touchscreen. Select the desired tab.

4. Enable Monopolar 1 footswitching at the local level by touching the button. The X is removed. Operation of handswitching or footswitching instruments is now enabled from the Monopolar 1 footswitching port.

5. Attach a two-pedal footswitch or three-pedal Valleylab mode-enabled footswitch to the monopolar footswitch receptacle on the back panel.

6. Attach any monopolar instrument to the Monopolar 1 instrument receptacle on the front panel.

7. Select the settings for the procedure.

8. Step on the appropriate footswitch to deliver monopolar power to the attached instrument.

Bipolar Setup InstructionsAutobipolar Function

Autobipolar allows the user to configure the system for automatic activation and cessation of bipolar energy.

1. Enable Autobipolar: Touch the wrench icon—Setup—Features—AutoBipolar, then exit the menus by touching the green arrows at the bottom of each screen.

2. When enabled, The Autobipolar button appears and the tab changes from Bipolar to Auto. Touch the Autobipolar button.

3. Select the power output mode: Low, Standard, or Macro.

4. Select the power output level by touching the green up and down arrows.

5. To change automatic activation parameters, touch the Setup button.

6. Set the desired parameters:

• Touch to restrict RF activation to use of a footswitch.

• Touch to enable RF activation without depressing a footswitch.• Below : touch the desired RF output delay value (seconds).• Below : touch the desired impedance value (ohm) at which RF energy will be discontinued.

7. Touch the green arrow to return to the Autobipolar touchscreen. The selected options and values are displayed.

8. Activate Autobipolar by closing the instrument tines or using a footswitch.

a. To use a footswitching LigaSure instrument connected to the LigaSure 1 instrument receptacle, attach the purple, seven-pin, LigaSure footswitch connector plug to the upper, purple LigaSure 1 footswitch receptacle on the rear panel.

b. To use a footswitching LigaSure instrument connected to the LigaSure 2 instrument receptacle, attach the orange, nine-pin, LigaSure footswitch connector plug to the lower, orange LigaSure 2 footswitch receptacle on the rear panel.

4. Connect the instrument or instruments to the LigaSure instrument receptacles on the front panel.

a. Instruments attached to the LigaSure 1 receptacle are controlled from the upper, purple section of the LigaSure touchscreen.

b. Instruments attached to the LigaSure 2 receptacle are controlled from the lower, orange section of the LigaSure touchscreen.

Note: The ForceTriad Energy Platform automatically sets all instruments to 2 bars.

Troubleshooting and DefinitionsDefinitions

Pure Cut provides a clean, precise cut in any tissue with little or no hemostasis.

Blend Cut is a conventional blended waveform that provides slower cutting and additional hemostasis.

Valleylab Mode is a unique combination of hemostasis and dissection and allows the user to slow down for more hemostasis and speed up for faster dissection. Thermal spread is equal or superior to Cut or Blend modes.

Coag Fulgurate coagulates tissue by sparking from the active electrode, through the air, to the patient tissue.

Coag Spray delivers wider fulguration; penetration is shallower and the affected tissue area is larger than with the fulgurate mode.

Bipolar Low delivers precision and fine control over the amount of desiccation.

Bipolar Standard is a conventional bipolar output at low voltage.

Bipolar Macro may be used for bipolar cutting or rapid coagulation. Power remains constant over a wide range of tissue types.

LigaSure Bar Settings shows as two green bars for all LigaSure instruments and is the default setting on the LigaSure instrument setting screen.

ForceTriad energy platform does not respond when turned on

Disconnected power cord or faulty outlet

Check power cord connections (ForceTriad energy platform and outlet). Connect the power cord to a functional outlet. (Do not connect the power cord to a power strip or extension cord.)

Faulty power cord Replace the power cord.

Fuse drawer is open or fuses are blown

Replace the blown fuse(s). Close the fuse drawer. Refer to the ForceTriad Energy Platform Service Manual.

Internal component malfunction

Use a backup ForceTriad energy platform. Contact your biomedical engineering department or contact a Covidien technical service representative for assistance.

System is on, but did not complete the self-test

Software malfunction Turn the system off, then turn it on.

ForceTriad energy platform is on and instrument is activated, but system does not deliver output

Malfunctioning footswitch or handswitching instrument

Turn off the ForceTriad energy platform. Check and correct all instrument connections.

Turn on the ForceTriad energy platform. Replace the instrument if it continues to malfunction.

Power is set too low Increase the power setting. Refer to section, Changing the Energy-Delivery Setting in the ForceTriad Energy Platform User’s Guide.

An alarm condition exists Note the code number and the information on the error screen and refer to System Alarms in the ForceTriad User’s Guide.

In case of a REM alarm, refer to Correcting a REM Alarm Condition in the ForceTriad Energy Platform User’s Guide.

Internal component malfunction

Contact your biomedical engineering department or contact a Covidien technical service representative for assistance.

System does not detect LigaSure instrument

Firmly insert the LigaSmart connector into the appropriate receptacle on the front panel. Ensure the tissue fusion touchscreen indicates that it has detected the instrument.

System does not detect monopolar instrument

Firmly insert the Smart connector into the appropriate receptacle on the front panel. Ensure the monopolar touchscreen indicates that it has detected the instrument.

System does not detect bipolar instrument

Firmly insert the connector into the appropriate receptacle on the front panel. Ensure the bipolar touchscreen indicates that it has detected the instrument.

The UNINTENTIONAL INPUT screen appears

Instrument buttons or footswitch is stuck, or something is making contact with the touchscreen

Check instrument buttons and footswitch pedals to ensure they are operational. Clear any objects making contact touchscreens away from the ForceTriad energy platform.

REM alarm illuminates red and enlarges, an audio tone sounds twice, and monopolar energy is disabled

Cord is improperly connected to ForceTriad energy platform

Verify that the patient return electrode cord is correctly connected to the ForceTriad energy platform.

Patient return electrode or cord is damaged

Inspect the plug, cord, and the connection of the cord to the patient return electrode. If you find evidence of excessive wear, cracks, breaks, or other visible damage, replace the patient return electrode and/or the cord.

Patient return electrode is improperly applied to patient

Verify that the patient return electrode is in contact with the patient, per the package instructions for applying the patient return electrode.

If the REM alarm persists apply another REM Polyhesive Patient Return Electrode. Refer to Correcting a REM Alarm Condition in the ForceTriad Energy Platform User’s Guide.

INVALID INSTRUMENT alert appears in touchscreen

Invalid instrument is connected

Disconnect invalid instrument from the Monopolar 2 port and attach it to the Monopolar 1 port. Refer to instrument’s instructions for use for compatibility instructions.

The UNKNOWN INSTRUMENT screen appears

The instrument does not have a barcode recognized by the current version of the ForceTriad software

Disconnect the instrument and replace it with a device with an instrument that has a barcode recognized by the software.

Update the ForceTriad energy platform’s software. Contact your biomedical engineering department or contact a Covidien technical service representative for assistance.

The CALIBRATION NEEDED screen appears

The ForceTriad energy platform requires recalibration

Consult the ForceTriad Energy Platform Service Manual for instructions regarding calibration.

The CHECK INSTRUMENT screen appears, six pulsed tones sound, and RF output is disabled (LigaSure function only)

Excessive tissue/eschar on electrode tips or jaws

Clean electrode tips and jaws with a wet gauze pad.

Electrodes have come loose from the instrument jaws

Re-insert the electrode into the instrument jaws making sure that all the electrode pins are firmly seated.

Metal or other foreign object is grasped within jaws

Avoid grasping objects, such as staples, clips, or encapsulated sutures in the jaws of the instrument.

Tissue grasped within jaws is too thin

Open the jaws and confirm that a sufficient amount of tissue is inside the jaws. If necessary, increase the amount of tissue and repeat the procedure.

Pooled fluids around instrument tip

Minimize or remove excess fluids.

The REACTIVATE screen appears, four pulsed tones sound (LigaSure function only)

The maximum seal cycle time has been reached

The system needs more time and energy to complete the seal. Reactivate the seal cycle without removing or repositioning the instrument.

The seal cycle was interrupted before completion

The handswitch or footswitch was released before the end tone activated. Reactivate the seal cycle without removing or repositioning the instrument.

Dual Instrument Error screen appears in the touchscreen

More than one instrument is connected under this touchscreen

Remove one of the instruments under the associated touchscreen.

The LigaSure Error screen appears requesting proper insertion of the two plugs

Connectors on LigaSure Advance instrument are switched or not making proper contact

Check the two plugs on the Advance instrument to ensure they are securely plugged into the correct receptacles. The LigaSmart connector should be attached to either LigaSure receptacle, and the Smart connector should be attached to the Monopolar 2 receptacle under the center touchscreen.

A footswitch function was attempted without enabling Monopolar 1 footswitching

Enable Monopolar 1 footswitching by touching the button on this screen with a green check mark.

The INVALID INSTRUMENT for Monopolar 1 screen appears

A function was attempted that is available only through the Monopolar 1 receptacle

Disconnect the instrument and attach it to the Monopolar 1 receptacle.

Continuous monitor interference

Malfunctioning monitor Replace the monitor.

Faulty chassis-to-ground connections

Check and correct the chassis ground connections for the monitor and for the energy platform.

Check other electrical equipment in the room for defective grounds.

Electrical equipment is grounded to different objects rather than a common ground. The ForceTriad energy platform may respond to the resulting voltage differences between grounded objects

Plug all electrical equipment into line power at the same location. Contact your biomedical engineering department or a Covidien technical service representative for assistance.

Abnormal neuromuscular stimulation (stop surgery immediately)

Metal-to-metal sparking Check all connections to the ForceTriad energy platform, patient return electrode, and active electrodes.

Can occur during coag Use a lower power setting for the fulgurate and the spray modes.

Abnormal 50 Hz-60 Hz leakage currents

Contact your biomedical engineering department or contact a Covidien technical-service representative for assistance.

Interference with other devices only when the ForceTriad energy platform is activated

Metal-to-metal sparking Check all connections to the energy platform, patient return electrode, and instruments.

High settings used for fulguration

Use lower power settings for fulguration.

Electrically inconsistent ground wires in the operating room

Verify that all ground wires are as short as possible and go to the same grounded metal.

If interference continues when the ForceTriad energy platform is activated, the monitor is responding to radiated frequencies

Ask your biomedical engineering department to check with the manufacturer of the monitor.

Some manufacturers offer RF choke filters for use in monitor leads. The filters reduce interference when the ForceTriad energy platform is activated and minimize the potential for an electrosurgical bur at the site of the monitor electrode.

Pacemaker interference Intermittent connections or metal-to-metal sparking

Check the active and patient return electrode cord connections.

It may be necessary to reprogram the pacemaker.

Current traveling from active to patient return electrode during monopolar electrosurgery is passing too close to pacemaker

Use bipolar instruments, if possible.

If you must use a monopolar instrument, place the patient return electrode as close as possible to the surgical site. Make sure the current path from the surgical site to the patient return electrode does not pass through the vicinity of the heart or the site where the pacemaker is implanted.

Always monitor patients with pacemakers during surgery and keep a defibrillator available.

Consult the pacemaker manufacturer or hospital cardiology department for further information when use of electrosurgical appliances is planned in patients with cardiac pacemakers.
