This part of the application refers to all conditions to be addressed at this school. Describe existing equal access condition (or conditions) that needs to be addressed, including any deficiencies with respect to the Americans with Disabilities Act (ADA) compliance and/or the Individuals with Disabilities Education Act (IDEA) requirements. Be specific about deficiencies and their impact on equal access to educational opportunities. Your narrative might include answers to questions similar to the following: Why and when was the condition identified? Is there a record of repeated concerns related to this condition? What is the urgency of this condition and why is this condition significant to the school? Is the condition listed in an ADA self-evaluation/transition plan? What was the process used to identify and prioritize the condition(s) to be addressed in this proposal? Was an ADA-compliance survey done, and if so, what was the method used? ABILITY TO IMPLEMENT PROJECTProvide information demonstrating that this is a well-planned and budgeted project that your District can accomplish. Address the following: Who/what was involved in the planning and decision-making process? Did people with disabilities and/or experience working with people with disabilities provide input? Does the District have the experience to implement a project of the proposed scope and complexity? Provide examples of past capital improvement projects similar in scope that the District, working with consultants and contractors as appropriate, has successfully completed.(150 words max.)
