We understand that Form 545 was mailed to your office January 22. This Form 545 was executed by the mother and we later learned from our chapter that the mother had completed it, inasmuch as the father and next of kin had died approximately two months after the service­man died overseas.

Although we gave our chapter worker specific instructions to advise the mother to submit certified copy of the father's death certificate, at the time she executed Form 5451 this appears not to have been done. We are, therefore, writing our chapter once more, advising them that such evidence is required, if Mrs. Nawracaj is to be recognized as next of kin.

Mrs. Nawra.caj, according to our chapter worker, is too emotio y upset to take the necessary steps to secure a certificate. t "tiai're&e:roe, hoped that our chapter will be able to . obtain your files. '

Jerome c. Nawracaj was the left waist gunner on a B-17 (Flying Fortress) bomber which departed fro~ Snetterton Heath, England on a bombing mission to Duren, Gemnny on 20 October 1943. The report reTeals that d~g this mission our planes encountered enemy aircraft. In the ensuing engagement Sergeant Uawracaj's plana became separated from the fomat.ion and was shot down. Six of the crewr parachuted to oatety and have .-~ince retunled to this country. : ..
