Key Points to Fill up the Form

1. Use English BLOCK CAPITAL letters only to fill the forms with neat & clean handwriting.

2. Residence ADDRESS : (a) Permanent address as per you ID proof

(b) Local Address is your current address where you live.

3. Colour PHOTOGRAPH: (a) Use Recent passport size color photograph only. (3.5 cms to 4.5 cms)

(b) Write your name & date of birth behind the photo

(c) Should be PASTED only on Original CAF form

4. SIGNATURE : (a) Cross signature on photo & it should not come over the face.

(b) All forms should be signed with same signature by applicant.

(c) All other supporting documents should be self-attested (signed by applicant).

5. For MNP-Portability (Changing Mobile Service Provider from other operator to Vodafone)

(a) Your existing number Last BILL should be PAID & photocopy of bill should be attached.

(b) Your existing number should be at least 90 days older with existing operator.

(c) Unique Porting ID/UPC is required. SMS "PORT (your 10 digit Mobile Number)" to 1900

6. J2J connect Scheme can be applied ONLY through JIO-JAC.
